Care Coordinator plays a critical role in improving trauma patient outcomes by identifying opportunities for system improvement and collaborating with multidisciplinary teams across the hospital. This position offers strong exposure to trauma program leadership, quality improvement processes, and national trauma standards while maintaining a stable day-shift schedule. It is an excellent opportunity for nurses interested in advancing into trauma program or healthcare quality leadership.

Full-Time - 1.0 FTE/80 Hours Biweekly

Day Shift - 0700-15:30

General Summary

Oversees the clinical care provided to the trauma patient in collaboration with the Medical Director, Trauma Services and the Trauma Program Coordinator. Ensures that nursing care and treatment is administered in accordance with physician instructions and in compliance with established standards, practice and regulatory requirements of the Pennsylvania Trauma Systems Foundation (PTSF) and other agencies.

Duties and Responsibilities

  • Assumes day-to-day responsibility for process/performance improvement activities as they relate to nursing and ancillary personnel.
  • Participates in morning rounds with the clinical team. Enters data of patient activities into the performance improvement database on a daily basis.
  • Participates in Care Management Rounds.
  • Coordinates trauma care management across the continuum of care, including planning and implementing of Clinical
  • Management Guidelines, monitoring of care in hospital patients and serving as a resource for clinical practice.
  • Assists, as time allows, in trauma team activation.
  • Responsible for the concurrent and accurate collection of clinical care data as defined by the Pennsylvania Trauma
  • System Foundation using PTSF database and performance improvement software.
  • Reviews medical records on a daily basis.
  • Presents educational lectures in a variety of forums within the institution and Trauma Nurse Course.
  • Identifies areas of trauma research to participate in clinical research.
  • Participates in injury prevention and outreach programs.

Common Expectations:

  • Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment.
  • Provides direct patient care to assigned patient age group(s).
  • Attends trauma conferences and maintains 18 hours of trauma-related category 1 CME credits per year.
  • Monitors policies, procedures and practice guidelines.
  • Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
  • Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications

Minimum Education:

  • Diploma Program Required or
  • Associates Degree Required and
  • Bachelors Degree Preferred

Work Experience:

  • 5 years Relevant RN experience required and
  • 2 years In critical care. Required and
  • Experience in Trauma, Emergency, or Critical Care Nursing. Required

Licenses:

  • Licensed Registered Nurse Upon Hire Required or
  • Registered Nurse Multi State License Upon Hire Required and
  • Certified Emergency Nurse Upon Hire Preferred or
  • Critical Care Registered Nurse - AACN Upon Hire Preferred and
  • Advanced Cardiac Life Support Upon Hire Required and
  • Ped Adv Life Support Upon Hire Required and
  • Basic Life Support Upon Hire Required

Courses and Training:

  • Trauma Certified Registered Nurse Certification Upon Hire Preferred
