Job Description

Main responsibility will be to manage patients in our well established Coumadin Clinic through in office Point of Care and Tele-management.  Experience preferred but not required.

Under general supervision, provides direct patient care at the beginner practitioner level, normally in an outpatient setting. Assesses patients, performs treatments, administers medications and instructs patients and their families in appropriate care. May require specialized skills and training depending on work area assignment. 

Duties and Responsibilities

  1. Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care.
  2. Competently uses point of care (POC) technology to obtain INR results (fingerstick blood results) that determine patient’s current therapeutic range.
  3. Provides in depth patient education at initiation of high risk medications and ongoing support to patients and caregivers. Appropriately organizes and prioritizes time based on patient condition and expected workload
  4. Is able to identify at risk patients and elevate issues using critical thinking. Manages high risk patients (such as patients with ventricular assist devices, immediate post surgery, and non-compliant patients
  5. Writes initial nursing histories, assesses patients' conditions and develops individual care plans for patients.
  6. Prepares patients for scheduled procedures to assist in patient assessment.
  7. Assists physicians with examinations, procedures and other processes related to direct patient care.
  8. Arranges for patients' ordered diagnostic and therapeutic services.
  9. Monitors and ensures accuracy of recording on patient medical records. Maintains all required reports, records, statistics, etc.
  10. Provides health care instruction/education to patient and family members.
  11. Performs a variety of nursing administrative duties.
  12. Assists attending physicians and aids patients and families as needed in times of stress or crisis.

Qualifications

  • Minimum Experience:
    • 6 - 12 months
  • Minimum Education:
    • Associate's Degree
  • Required Certification:
    • Registration and current RN license in PA; Basic Life Support (BLS) certification
  • Preferred Experience:
    • Anti-Coagulation Management experience preferred but not required
