On call 4:30pm until 8am

Every other weekend on call requirement

2 Holidays per year

General Summary

Provides professional nursing services to individuals in the home through a process of nursing assessment, planning, care and evaluation in accordance with physicians' orders, the established plan of care, the patient's diagnosis, corporate policy and procedures and the Nurse Practice Act. Carries primary care management functions for assigned patients.

Duties and Responsibilities

Essential Functions:

  • Assesses the physical, psychosocial, spiritual, and environmental needs of assigned patients on the initial visit and reassesses same on each subsequent visit.
  • Develops a written plan of care in accordance with the patient's diagnosis and family needs, physician orders and the corporate policies and procedures. Coordinates the development of this plan with other disciplines providing care for patient.
  • Uses appropriate disciplines and community resources for optimum patient care.
  • Modifies the written plan of care as necessitated by changes in patient's condition.
  • Notifies physician and other care agencies of changes in patient's condition and care needs and obtains new orders, as needed.
  • Administers nursing care in accordance with the established plan of care including, but is not limited to, therapeutic treatment, teaching, counseling and supervising patient and/or care givers in performing specific procedures.
  • Performs case management functions, when assigned as primary clinician.
  • Participates in patient care conferences with other members of the interdisciplinary team and recommends patients for such conferences. Documents results, when functioning as the patients primary clinician.
  • Documents observations, assessments, interventions of visits, phone calls and conferences in the patient's clinical record on a timely basis and legible manner.
  • Discharges the patient when appropriate, notifying the physician and manager. Instructs the patient and/or family how to access necessary follow-up.
  • Provides both direct care and telephone consultation on a rotating basis, twenty-four hours a day.
  • Provides clinical supervision to the home health aides, licensed practical nurses or volunteer staff, for assigned patients and provides input regarding their performance to the appropriate manager.
  • Assists in the orientation of new staff or continuing education of existing staff as requested.
  • Participates in the student nurse program, as delegated.
  • Variation of duties may result based on the assigned customer care unit(s). This may include differences in the specific clinical skills required of the RN. Unit based skill lists outline expectations beyond the house wide RN requirements. SYS-NURS-L Nursing Education Requirements Policy outlines current certification(s) required for specific areas of nursing practice and is considered to be part of the job description.
  • In addition to requirements specified in the license and certification section below, current BLS certification for all areas of nursing practice is required.Current certification may be required for the following areas of nursing practice:Emergency Care Center - ACLS, PALSIntensive Care Center & MSCC Lead - ACLS, PALSL&D - ACLS, NRP, - Experienced L&D nurses will obtain within 6 months and new L&D nurses will obtain at 1 year of experienceNICU - NRPPEDS - PALSPCC/PACU/ENDO - ACLS, PALSProcedural - ACLS

  • Functions in a cost-effective manner in regard to utilization of time, mileage, supplies, etc.
  • Meets or exceeds established productivity and quality assurance standards.
  • Seeks consultation, guidance and direction from professional or administrative staff, as needed.
  • Informs manager of perceived problem areas related to patient care, corporate management, or community relations.
  • Keeps clinical skills current with nursing practice norms.
  • Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
  • Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
  • Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
  • Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
  • Represents the VNA of York County at community activities, as delegated.

Qualifications

Minimum Education:

  • Diploma Program Required or
  • Associates Degree Required

Work Experience:

  • 1 year Relevant experience. Required

Licenses:

  • Licensed Registered Nurse Upon Hire Required or
  • Registered Nurse Multi State License Upon Hire Required and
  • Basic Life Support Upon Hire Required

Driver's License Statement:

Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
