Registered Nurse - Diabetes Educator

Location: WellSpan Health, Stevens, PA
Schedule: Full Time

Full-time Day Shift

80 hours bi-weekly

8:00am - 4:30pm - with variations

Diabetes Educator has evening classes up to 8:30pm

Occasional weekend commitments for community benefit fairs

Employee would be remote capable but primarily on-site

Position will also have an expectation of supporting our sister site of Lebanon (Good Sam) Diabetes Education department for providing cross-coverage and broad service coverage

Occasional travel is expected when needed to sister site or to a community benefit fair

CDCES or certified education preferred but not required.

General Summary

Provides group and individual Diabetes Education. Provides nutritional counseling to clients referred by a provider. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan with each program participant.

Duties and Responsibilities

  • Provides group education and individual education in Diabetes Education.
  • Provides nutritional counseling to clients referred by a provider.
  • Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.
  • Manages blood glucose for patients.
  • Manages insulin pumps and continuous glucose monitors for patients.
  • Collects program participant assessment data, using the ADDE7 framework, in a collaborative and ongoing manner.
  • Collaboratively develops an individualized follow-up plan with each program participant.
  • Conducts a follow-up assessment upon completion of program services, using outcome measures from the AADE7 Continuum of Outcomes Framework.
  • Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records.
  • Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
  • Participates in peer review process to evaluate performance of other professional staff.

  • Enhances professional growth and development through participation in educational programs, current literature, conferences, and workshops.

Qualifications

Minimum Education:

  • Associates Degree Registered Nurse Required
  • Bachelors Degree Preferred

Work Experience:

  • 2 years Relevant experience. Required
  • Med/Surg, ICU, Care Management, and Diabetes Education experience preferred

Licenses:

  • Licensed Registered Nurse Upon Hire Required or
  • Registered Nurse Multi State License Upon Hire Required or
  • Licensed Dietitian-Nutritionist Upon Hire Required and
  • Heartsaver CPR/AED Upon Hire Required
  • Certified Diabetes Care and Education Specialist within 3 years Preferred

Knowledge, Skills, and Abilities:

  • Able to lead and manage groups.
