General Summary


Under the direct supervision of the Senior Director of Heart and Vascular Services at York Hospital and the general supervision of the Cardiology practice and the Heart and Vascular leadership team, coordinates patient care/services in the inpatient setting including the transition of inpatient transfers from another acute care facility and the transition to the ambulatory setting post discharge to improve patient outcomes.  


Duties and Responsibilities



  1. Introduces patient to the WellSpan Cardiology Program.
  2. Serves as a liaison between the patients, families and healthcare team during hospitalization and the immediate post- discharge period.  In conjunction with patient logistics coordinates with transferring facilities to ensure an organized transfer and communicates pertinent patient information to all clinicians involved in the care of the patient upon receiving them at York Hospital.
  3. Communicates to Cardiologist, APP’s, attending physician, and hospital staff regarding abnormal findings in the evaluation of patients.  Conducts chart reviews to ensure guideline directed medical therapy is applied.
  4. Works with the hospitalists, cardiologists, and APPs in assessing readiness for procedures, discharge, and risk for readmission.
  5. Responsible for the guidance and coordination of care for patients and their caregivers through the continuum of care in the inpatient setting and transfer to the outpatient setting.
  6. Provides individualized information, support, and education. 
  7. Works with cardiology team and inpatient resources to coordinate procedures, consults, and follow up appointments. Identifies and elevates gaps in social determinates of health to appropriate resources.
  8. Participates in interdisciplinary nursing rounds for inpatients (discharge planning process).
  9. Participates in PI and other metrics to as determined by program needs.
  10. Maintains a thorough knowledge and understanding of evidence based/best practice.
  11. Revises and monitors educational materials to ensure that they are coordinated, understandable and meet the program standards.
  12. Protects patient confidentiality by promoting professional staff communications.
  13. Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
  14. Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population.  Assesses and interprets patient age specific data and provides appropriate, age specific treatment.  Provides direct patient care to assigned patient age group(s).
  15. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  16. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.





  Minimum Education      Diploma RN

  Preferred Education      Bachelor’s Degree

  Minimum Experience     3 - 5 years cardiology nursing experience; procedural experience preferred

  Req Cert/Registration    Registration and current RN license in PA; BLS certification

  Pref Cert/Registration   Certification in appropriate nursing specialty; Procedural cardiovascular nursing experience preferred
