Job Description:

Under the direct supervision of the Medical Director Continuing Care Services, provides leadership and expertise in implementing and maintaining system-wide, comprehensive Palliative Care programs for WellSpan Health. Develops close partnerships with stakeholders across the system and with hospice agencies serving geographical areas to enhance hospice services for patients in WellSpan service areas. Works collaboratively with WellSpan Medical Group, Services Lines, Acute Care Entities, Case Management, Home Health Care, Population Health, and others to execute tactics that advance system’s strategic plan.


Duties and Responsibilities


Demonstrates knowledge of the principles of Palliative Care.

Develops close partnerships with stakeholders across the system and with hospice agencies serving geographical areas. Uses a multidisciplinary approach to accomplish efficiencies through strong communication with case management and other disciplines.

Develops operational strategies and procedures to achieve palliative care initiatives and outcomes.

Oversees continuous quality improvement and uses extensive knowledge, experience, program outcomes, and research to identify problem situations or issues in systems of care.

Evaluates metrics of Palliative Care Dashboard in support of determining successes and opportunities for improvement.

Collaborates and coordinates with programs under the Continuing Care Services umbrella such as Advanced Illness Management; Horizon Planning; Hospital at Home; and internal and external hospice entities.

Works collaboratively with the Palliative Care Practice Manager and Medical Director to expand palliative care services across the system and increasing referral base.

Demonstrates knowledge and leadership of documentation and billing practices for revenue management and unique payer agreements.

Assists with the evaluation of marketing and educational needs for standardization across system

Designs and integrates focused foundational/fundamental Palliative Medicine curriculum for residents rotating through critical care areas

a. Systemwide nursing and interdisciplinary provider/staff education

Designs and implements community Teaching, presentations, and Educational materials to raise awareness and promote Palliative Care.

Works collaboratively with WellSpan Medical Group and other WellSpan entities’ leadership teams to write and revise protocols related to Palliative Care within WellSpan Health.

Responsible for Accreditation and Palliative Care Certifications as applicable.

Responsible for budget development and adherence in conjunction with Practice Manager and Medical Director.





3-5 Years Palliative Care, Advance care planning, chronic disease management, care coordination;

Experience in leading multi-site system wide projects; Project management and leadership experience.

1-3 years working clinically or administratively within Palliative Care and/or Hospice

Bachelor's Degree in healthcare, nursing, or related field.
