Position Summary:

General Summary:
Under supervision of the Chief Medical Officer, is responsible for Medical Group wide patient safety programs and initiatives. Inspires our physicians, advanced practice clinicians (APCs), and staff to achieve a level of commitment and reliability that results in an interdisciplinary approach to current and potential problem analysis. Serves as the organizational steward to define our culture of safety while focusing on the needs of our patients and their families. Improves safety by persisting that all harm is preventable. Maintains a high level of competency in the science of safety and improvement and leads the Medical Group to become a high reliability organization by rigorous adherence to those principles. Serves as the patient safety conscience of the organization and holds us accountable to first do no harm. Works in close collaboration with the Corporate Director of Patient Safety, Infection Control, as well as physicians and staff in all medical group facilities. Maintains compliance with all medical group and system patient safety policies and procedures as well as any regulatory and legal requirements.

Duties and Responsibilities:
1 Fosters the use of the occurrence reporting system throughout the Medical Group to minimize blame and retribution with an emphasis on system improvement. Assists with the organization of the Daily Safety Huddle Report and distributes as appropriate. Elevates concerns as needed.
2 Coordinates and disseminates education and information to all Medical Group employees in regards to patient safety and infection control improvements. Elevates concerns as needed.
3 Investigates all serious events and identified incidents. Ensures all necessary actions, due to any investigation, are promptly undertaken. Reports any actions taken to the Clinical Director of Quality & Innovation, the Chief Medical Officer, the Board of Directors, and other committees as needed.
4 Coordinates with infection Control to perform surveillance for healthcare associated infections.
5 Trends and analyzes data on safety events and disseminates the data to improve patient safety across the continuum of care.
6 Attends system level Employee Safety Committee meetings as the Medical Groups representative.
7 In conjunction with staff from the Medical Group, Quality Management, and Risk Management departments, employs systematic techniques such as Failure Mode Effects Analysis (FMEA) and Root Cause Analysis (RCA) for the investigation of safety failures of the Medical Group.
8 Operates and maintains event reporting databases, data integrity and data collection methodologies, assuring accuracy and completeness of the data.
9 Assesses the Medical Groups culture of safety using industry standard assessment tools. Works with other members of leadership to develop and implement programs that seek to address issues of concerns from these surveys.
10 Develops, implements and coordinates patient safety initiatives and goals such as Institute for Healthcare Improvement (IHI), Pennsylvania Department of Health, and the Centers for Disease Control (CDC).
11 Reports WellSpan Health Patient Safety information to designated committees and boards as directed by senior leadership.
12 Attends and assists to manage the agendas for the Medical Groups Patient Safety Committee meetings. Provides consultation for the committee to foster adoption of system-wide patient safety and infection control initiatives.

13 Monitors the external environment for best practices and evaluates these for practice implementation.
14 Provides patient safety related training as needed, including such topics as Just Culture, root cause analysis, event reporting and management, etc.
15 Represents the Medical Group in additional safety meetings, such as radiation and laser safety, applies principles as needed.

Required Skills
  • Excellent written and oral communication skills
  • Excellent organizational skills
  • Knowledge of state and national patient safety regulations
Required Experience
  • Bachelor's Degree is required
  • 3-5 years clinical experience
  • Experience in risk management, performance improvement and patient safety are preferred
