Position Summary:

General Summary:

Under the general supervision of the Patient Safety Officer, promotes an environment of safe care for patients by supporting patient safety initiatives. Works with leadership, healthcare providers, and hospital staff to analyze and improve clinical care practices. In accordance to applicable regulatory and legislative requirements ensures completeness and timeliness of patient safety reported data. Promotes and guides the organization towards a Just Culture Model.

Duties and Responsibilities:


   1  Reviews and prepares patient safety events reported in the hospital’s safety reporting system for submission to Pennsylvania Safety Reporting System (PSRS).

   2  Ensures timely reporting of all required events to PSRS.  

   3  Advocates the effective use of the safety reporting system to minimize blame and retribution and emphasize systems improvements.

   4  Operates and maintains safety reporting system ensuring accuracy and completeness of data.

   5  Provides and advises hospital staff of reporting system capabilities and functionality.

   6  Collaborates with Patient Safety Officer to identify and facilitate necessary practice changes based on clinical and administrative data and current research findings.

   7  In collaboration with the Patient Safety Officer and Senior Leadership, participates in patient safety initiatives, such as Root Cause Analysis (RCA), Focused Discussions and Failure Modes and Effects Analysis (FMEA) for investigation of patient safety events.

   8  Supports the hospital patient safety program through facilitation of the Daily Safety Brief.

   9  Drives and supports the organization towards a Just Culture Model.

11  Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

  13  Maintains appropriate records, reports, and files as required.

The following are nonessential job accountabilities for ADA purposes:

14  Attends meetings as required, and participates on committees as requested.

      Performs other related duties as identified.

Required Skills


Minimum Education             Bachelor's Degree

Minimum Experience         

3 - 5 years  Allied Health experience, computer science, mathematics, in this role the ideal candidate will need to be very proficient in utilizing Excel and data manipulation. 

Pref Cert/Registration          Registration and current RN license in PA

Excellent interpersonal and written and oral communication skills; Excellent organizational skills; knowledge of state and national patient safety regulations
