General Summary

Coordinates, communicates and predicts impacts to bed management, transfers and admissions from all possible points of entry and provides general oversight of patient placement and patient flow throughout the Hospital/Health System.

Duties and Responsibilities

Essential Functions:

  • Provides patient flow coordination, management and optimization 24/7 and coordinates all associated activities with Physicians, Providers and other Clinical Personnel, to ensure timely patient placement based on the patient's care needs, diagnosis and acuity.
  • Promotes cooperation and effectively communicates to appropriate professionals throughout the hospital/health system to achieve the objectives of the patient logistics' department regarding the improvement of patient flow and urgency for placement and movement of patients during elevated capacity situations.
  • Prioritization and utilization of guidelines to accurately predict and manage patient flow based on the needs, diagnosis and acuity of all patients through all points of entry within the facility and Health System.
  • Coordinate and facilitate direct admissions and transfers, including making appropriate transportation arrangements for the smooth transition and coordination of care.
  • Uses exceptional clinical judgment, based on acquired nursing skills, knowledge base and education to assess patient's care delivery needs, acuity, diagnosis and required specialty services in relation to unit placement and available clinical resources.

  • Coordinates and integrates services within the department/procedural areas/entity, with other departments/procedural areas/entities, and with the System's primary functions.
  • Continuously assesses and improves the department/entity/System's performance. Maintains appropriate quality control/assessment programs (if applicable).
  • Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
  • Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
  • Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications

Minimum Education:

  • Associate's degree in nursing required
  • Bachelor's degree in nursing (BSN) preferred

Work Experience:

  • 2 years clinical nursing experience in a highly paced acute care environment required and
  • Supervisory and/or charge experience considered essential required

Licenses:

  • Licensed Registered Nurse Upon Hire Required or
  • Registered Nurse Multi State License Upon Hire Required

Knowledge, Skills, and Abilities:

  • Typing and moderate computer skills and knowledge preferred.
