PRN Chaplain Ephrata
Job Description:
Under the general supervision of the Director-Spiritual Care & Education, provides spiritual counseling and guidance to patients, families and staff and performs or arranges for religious services to meet patient needs.
Duties and Responsibilities
- Participates in a spiritual assessment of the patient/family/caregivers, as indicated.
- Facilitates the development of a plan of care to meet the identified spiritual needs in conjunction with the interdisciplinary team.
- Provides direct spiritual care to patients/families/caregivers/staff, when needed.
- Serves as liaison to community chaplains and spiritual counselors, as indicated.
- Documents all patient/family/caregiver contacts and provides additional written, pertinent information as indicated.
- Participates in patient care conference and interdisciplinary team meetings.
- Provides consultation, education and support to the interdisciplinary team on spiritual care.
- Provides for funeral or memorial services or other services as requested by patient/families.
- Provides bereavement support.
- Provides spiritual support to staff.
Qualifications
- Notes:
- Other combinations of formal education, training and experience may be considered.
- Minimum Experience:
- 2 - 3 years
- Minimum Education:
- Bachelor's Degree
- Required Certification:
- Ordained/Endorsed by recognized religious body/proper ecclesiastical authority; Certified (or eligible) Chaplain by Association of Professional Chaplains (APC)
- Required Courses:
- Graduate of accredited seminary or school of theology
