General Summary

Supports efforts to implement care delivery systems that are safe, timely, effective, equitable, efficient and patient centered. Works with the Practice Managers and Site Directors to advance and standardize clinical operations throughout the system, within a culture of quality, safety and efficient care. Assists the Director of Clinical Operations in management of staff to deliver high quality, patient-centered care. Assists with directing and coordinating system and operational priorities. Provides leadership and oversight of the Clinical Supervisors. Develops and initiates standardized nursing education for the service lines, in line with System Practice and evidenced based protocols.

Duties and Responsibilities

Essential Functions:

  • Works as a member of the clinical operations leadership team to develop and ensure the successful completion of service line goals, objectives and system aims for improving clinical operations of high quality and patient safety.
  • Provides supervision, direction, oversight and support of the work of Clinical Supervisors in the practice and across transitions of care.
  • Responsible for hiring, performance review of Clinical Supervisors.
  • Collaborates with Practice Managers to provide performance evaluation and clinical competency review of clinical staff in the practices.
  • Works with the Care Coordination Leadership Team to integrate and coordinate PCMH care management activities longitudinally across the system.
  • Supports the advancement of the System's Accountable Care Strategies.
  • Ensures alignment of clinical policies and patient care activities within the service line with other System entities through service on the WellSpan Nursing Executive Council.
  • Collaborates with other System departments and Service Lines to develop care standards and integrate clinical quality initiatives in areas such as patient and population management, quality improvement, infection control, patient safety, care management and information technology.
  • Works with the leadership team to develop appropriate staffing models to support quality, cost effective patient care in an accountable care environment.
  • Assists in the implementation, monitoring, auditing, and ongoing improvement of quality initiatives. Tracks the progress of goals, productivity, and performance standards. Stays abreast of emerging trends and state/national guidelines and standards related to clinical quality and health services.
  • Encourages physicians and physician practice leadership and staff to explore the benefits, challenges, and opportunities in improving health outcomes and patient experiences while containing healthcare costs. Assists in avoiding duplication of efforts, minimizing inconsistencies, recognizing and reducing burden on the participants, and developing and deploying strategies that are cost effective and improve EHR utilization and standardization.
  • Continuously monitors and manages the practice transformation process, including but not limited to practice observations of current state, review of findings and concerns, consultation of best practice recommendations, and opportunities for improvement related to optimized clinical workflows.
  • Works and coordinates with practice leadership, providers, and clinical staff to operationalize strategies to improve, including implementation and training of efficient and standardized clinical workflows, as well as efficient use of health information technology to support patient-centered care delivery.
  • Identifies gaps in clinical workflows or processes and works with practice team and operational leadership to achieve expected outcomes.

Qualifications

Minimum Education:

  • Bachelors Degree Required

Work Experience:

  • 3 years In healthcare leadership and Medical Group practice experience. Required
  • Lean Daily Management experience. Preferred

Licenses:

  • Licensed Registered Nurse Upon Hire Required or
  • Registered Nurse Multi State License Upon Hire Required and
  • Board Certified in Ambulatory Care Nursing within 2 years Preferred or
  • Certificate in Ambulatory Care Nursing within 2 years Preferred
