Infusion Intake Coordinator - Full-Time - Day

M-F: 8am-4:30pm or 8:30am-5pm

General Summary

Processes all information and referral calls for intake. Provides support to multiple referral sources. Ensures all the information for onboarding the patient is complete prior to beginning services. Assists with coordination of care and/or transfers to nursing agencies and other outside services. Performs a variety of functions including, but not limited to, account follow up, answering inquiries, resolving problems and interacting with home health agencies, and case management.

Essential Functions:

  • Provides guidance and support as appropriate at the time of initial referral contact, and assesses for appropriateness for home infusion.
  • Elicits accurate and pertinent information regarding the patient's referral (demographics, start of care, initial order and past medical history).
  • Identifies and evaluates patient and family needs.
  • Secures information relative to patient's financial status or insurance coverage. May complete insurance forms, verify insurance coverage, and secure other information if needed.
  • Functions as a liaison between referral source and WellSpan Infusion Service.
  • Receives information on patients to be discharged from facilities and schedules start of care (i.e. nursing and delivery).
  • Inputs initial information into computer to begin the intake process and create initial patient packet.
  • Forwards insurance information to the Authorization/Insurance Specialist to determine eligibility and benefits. Monitors Insurance verification process to ensure timely hand off for referral to pharmacy teams.
  • Responds to requests for information regarding services from patients, families, agencies, hospitals, physicians, etc.
  • Directs caller to another source of care if WellSpan Infusion Services is unable to supply the service requested.
  • Interprets services to callers.
  • Develops relationships with Case Management and referral sources.

Common Expectations:

  • Maintains established policies and procedures, objectives, quality assessment and safety standards.
  • Participates in educational programs and in-service meetings.
  • Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications

Minimum Education:

  • High School Diploma or GED Required

Work Experience:

  • 1 year Relevant experience Required
  • Home Infusion Intake; Insurance Billing Experience Preferred

Knowledge, Skills, and Abilities:

  • Excellent interpersonal/communications skills
  • ICD10
  • Insurance knowledge
  • Computer skills
  • Customer service skills preferred
  • Able to work independently
