RN Diabetes Educator - Lebanon
Job Description:
Under the general supervision of the Service Line Administrative team or Practice Manager, provides group and individual Diabetes Education. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan with each program participant.
Duties and Responsibilities
- Provides group education and individual education in Diabetes Education.
- Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSMT program participants.
- Collects DSMT program participant assessment data, using the ADDE7 framework, in a collaborative and ongoing manner.
- Collaboratively develops an individualized follow-up plan with each program participant.
- Conducts a follow-up assessment upon completion of DSMT program services, using outcome measures from the AADE7 Continuum of Outcomes Framework.
- Documents assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records.
- Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
- Participate in peer review process to evaluate performance of other professional staff.
- Able to lead and manage groups.
Qualifications
- Minimum Experience:
- 2 – 3 years
- Registered Nurse a major plus
- Minimum Education:
- Bachelor's Degree
- Required Certification:
- Registration: Certified Diabetes Care and Education Specialist preferred
