Job Description:

Under direct supervision of the Program Manager-Horizon/Advanced Care Planning, provides support for implementing and maintaining system-wide, comprehensive Horizon Planning programs for WellSpan Health. Horizon Planning and Horizon Plan are trademarks of WellSpan Health.


Duties and Responsibilities


  1. Assists the Program Manager in partnering with stakeholders across the system to establish Horizon Planning goals and to achieve desired results.
  2. Performs data analysis for Horizon Planning program initiatives and outcomes. Uses metrics to drive modification and improvements to the program. Reports program progress to key stakeholders. Provides analysis and critical evaluation of program growth opportunities.
  3. Coordinates the enrollment, registration, and documentation for Certified Facilitator trainings in Skilled Nursing Facilities in a four County area. Documents all key aspects of grant implementation of training sessions. Assists in developing and implementing the organizational structure for Horizon Planning trainers and facilitators as outlined in the grant.
  4. Assists the Program Manager in coordinating continuing education and training for all members of the healthcare team on the importance and requirements of advance care planning, instruction on how to elicit healthcare goals and preferences, and how to document healthcare choices.
  5. Collaborates with research consultants and the Program Manager on the research design and protocols, and collects all data needed to evaluate effectiveness of grant outcomes. Makes recommendations for practice improvement and collaborates to ensure high quality of care is delivered.
  6. Works closely with clinical and medical informatics team to leverage technology to support patient participation, clinician documentation, and storage and retrieval of Horizon Planning related documents through the Electronic Medical Record (EMR). Performs analysis of data to ensure integrity of the Horizon Planning process of documentation and scanning.
  7. Assists in the coordination, delivery, and evaluation of educational programs for Horizon Planning within WellSpan Health and the local community.
  8. Participates, as needed, in implementing a community education plan. Works with community groups, professional organizations, patient partners, and volunteers to raise awareness and promote Horizon Planning.
  9. Works with the marketing team, in consultation with the Program Manager, to develop and maintain content for WellSpan Health external and internal Horizon Planning websites.




  • Minimum Experience:
    • 5 - 7 years


  • Minimum Education:
    • Master’s Degree in field of Social Sciences, Nursing, or Public Health
    • Other combinations of formal education, training and experience may be considered


  • Physical Demands:
    • Extensive travel required


  • Skills:
    • Data collection and analysis; Training in the theory and practice of advance care planning; Experience working on multi-site system wide projects; Project management experience, Advance care planning, chronic disease management, care coordination, and/or palliative care a plus but not required
