Job Description
Under the general supervision of the Foundations Pregnancy Support Services - Program Manager, collaborates with all members of the Foundations care team, internal WellSpan programs, and community agencies where patients live. The community health worker will work closely with pregnant and postpartum women who have a history of substance use along with their children through weekly outreach through telephone calls/texting and home visits. The community health worker advocates extensively on behalf of patients and families to gain access to services and resources by building trusting relationships through a non-judgmental, compassionate, and caring approach. The community health worker will work closely with our pregnant and postpartum mothers to empower them and their children for resilient futures by helping mothers connect and navigate the surrounding communities based on the family’s social determinates of health needs.
Duties and Responsibilities

Assesses needs of population, patient or individual on an ongoing basis. Identifies barriers to care and makes appropriate recommendations for follow-up or referral.

Utilizes Trauma Informed Care as a family-centered approached in self-management related to services needed, substance use disorders, and helps empower the woman and her children for healthier futures.

Utilizes Motivational Interviewing techniques as a patient-centered approach to activate patients in self-management related to social determinants of health.

Educates community members, either individually or in groups, about how to access and/or use the health care system. Shares relevant concerns from community members with medical providers or others within the health care system.

Identifies and utilizes culturally appropriate outreach strategies to provide specific health promotion and disease prevention education to specific population groups. This may include but is not limited to developing and coordinating a network of community lay leaders, implementing community education and screening events, and conducting relevant follow-up with participants.

Assists, as requested, in the coordination of care for clients/patients. This may include but is not limited to facilitating enrollment in public health insurance and charity care; conducting site visits with at-risk patients/clients and providing a cultural context for effective medical care and behavior change as part of a clinical care team.

Maintains a caseload of patients according to department policies.

Evaluates program outcomes/service effectiveness through tracking participation levels, conducting participant surveys, reviewing chart records, and engaging patient in regular follow-ups.

Documents and maintains accurate and confidential records of client interventions.

Actively participates on community coalitions and boards as an advocate for specific populations and as a liaison for WellSpan Health.

Attends appropriate trainings and certifications assigned by Program Manager
Qualifications

Minimum Experience:

One year in a community related position
