Care Coordinator - Oncology Nurse Navigator (RN)

Position Summary:

General Summary:

Under general supervision, RN Care Coordinator coordinates all aspects of treatment, evaluation, support services and education for cancer patients.  Coordinates clinical documentation from referring and treating physicians for each patient.

Duties and Responsibilities:

The following are essential job accountabilities:

 1 Introduces patient to the Health System.

 2 Assesses patient needs and barriers to care at first encounter and reassesses throughout navigation period.

 3 Provides holistic, consistent care.

 4 Serves as a consistent liaison among patients, families, caregivers, and providers to optimize patient outcomes.

 5 Provides individualized care, education, and support.

 6 Provides guidance and coordination of care to patient and their caregivers.

 7 Coordinates appointments as needed.

 8 Reinforces physician instructions as appropriate.

 9 Reports outcome of nursing/patient care/coordination activities to Oncology Service Line leadership.

 10 Demonstrates familiarity with clinical treatment guidelines and resources throughout the disease process.

 11 Collaborates with research team in clinical trials enrollment and data collection.

 12 Identifies patient and/or provider needs and develops quality programs to meet those needs.  Ensures the delivery of quality services for patients.

 13 Revises and monitors educational materials to ensure that they are coordinated, understandable, and meet program standards.

 14 Protects patient confidentiality by promoting professional staff communications.

 15 Collaborate with leadership to provide program evaluation, improvement, tracking metrics, and patient outcomes and report findings to Cancer Committee

Required Skills

Bachelor's Degree in Nursing 

Minimum 3 years nursing experience, at least 2 are oncology related

Current RN license in PA

OCN® (Oncology Certified Nurse) preferred
