Day Shifts


General Summary


Under general supervision, coordinates patient care/services from the inpatient setting to an outpatient setting in order to improve patient outcomes.


Duties and Responsibilities


  1. Introduces patient to the VNA.
  2. Serves as a liaison between the patients, families and healthcare team during hospitalization and the immediate post- discharge period
  3. Communicates to appropriate personnel regarding abnormal findings in the evaluation of patients
  4. Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment.
  5. Provides individualized information, support and education.
  6. Works with appropriate discharge planning to coordinate VNA referral/admission visits.
  7. Reinforces and assesses pre- and post-operative instructions and provides further education as appropriate.
  8. Responsible for the guidance and coordination of care for patients and their caregivers through the continuum of care.
  9. Responsible for PI and other metrics to determine effectiveness with all level of care providers to optimize the patient care experience.       
  10. Maintains a thorough knowledge and understanding of evidence based/best practice.
  11. Revises and monitors educational materials to ensure that they are coordinated, understandable and meet the program standards.
  12. Protects patient confidentiality by promoting professional staff communications.

  2. Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population.  Assesses and interprets patient age specific data and provides appropriate, age specific treatment.  Provides direct patient care to assigned patient age group(s).
  3. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  4. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.




  Minimum Education      Bachelor's Degree

  Minimum Experience     3 - 5 years nursing experience

  Req Cert/Registration    Registration and current RN license in PA

  Pref Cert/Registration   Certification in appropriate nursing specialty

  Working Conditions       Occasional exposure to unpleasant patient or unit elements

                                  Subject to varying and unpredictable situations

                                  Handles emergency or crisis situations

                                  May perform emergency care

                                  Subject to many interruptions

                                  Travel required

                                  Occasional pressure due to multiple calls and inquiries

                                  Occasionally exposed to the risk of blood borne diseases
