Job Description

Coordinates patient care in the outpatient setting to encompass a thorough continuum of quality cardiovascular care.

Duties and Responsibilities

  1. Introduces patient to the Health System.
  2. Serves as a liaison between the patients, families, and healthcare team throughout the process of diagnosis and treatment.
  3. Compiles patient’s current health record information, with investigation into current relevant test results, medications, Lab results, and health history. Preps charts prior to office visit, and shares with EP and Cardiologist to ensure best plan of care for patient. The chart preparation includes records retrieval for both the ICVD clinic and EP clinic.  May include retrieval for family members where warranted.
  4. Assists physician with pt office visits, navigating EMR, orders and functioning as a physician/ patient advocate during the visit.  Performs blood draws and processing of Invitae genetic testing when ordered.
  5. Works to assist patients in coordination of the plan of care for ICVD clinic patients across the WellSpan system, including proper preparation of pts for tests and procedures, as well as follow up care.
  6. Primary focus on Inherited Cardiovascular Disease and electrophysiology Clinic with possibility of assisting with general cardiology as needed.  This provides ability to expand Cardiology services to all 3 office locations.
  7. Ensures the timely delivery of quality services for patients, including proper prior authorization of tests/procedures and medications.
  8. Establishes, monitors, and revises educational materials for patient teaching. Provides individualized educational materials to patients and families for medications, diagnosis, tests/procedures and prevention strategies.
  9. May be responsible for PI and other metrics to determine effectiveness, quality, and safety of the patient’s experience, as well as possible participation in quality programs.
  10. Thorough knowledge of evidence based / best practice.
  11. Triaging of pt phone calls, and patient communication of test results.  Performs Pre and post procedural calls and follow up. 
  12. Assists providers with in-basket message clarifications.
  13. Assists with medication refills, and medication prior authorizations.
  14. Protects patient confidentiality by promoting professional staff communications.

Qualifications

  • Minimum Experience:
    • 3 - 5 years acute cardiovasular care experience
  • Minimum Education:
    • Bachelor's Degree and Current RN license in PA
