General Summary:

Under general supervision, coordinates all aspects of treatment, evaluation, support services and education for high-risk benign lesions and breast cancer patients.  Coordinates clinical documentation from referring and treating physicians for each patient.

 

 

The following are essential job accountabilities:

  1. Introduces patient to the Health System.
  2. Assesses patient needs and barriers to care at first encounter and reassesses throughout navigation period.
  3. Performs mammography studies; obtains patient history; explains routine procedures and addresses patient concerns.
  4. Provides holistic, consistent care.
  5. Serves as a consistent liaison among patients, families, caregivers, and providers to optimize patient outcomes.
  6. Provides individualized care, education, and support.
  7. Provides guidance and coordination of care to patient and their caregivers.
  8. Coordinates appointments as needed.
  9. Reinforces physician instructions as appropriate.
  10. Is a member of the breast care team which begins at biopsy through diagnosis and treatment. Responsibilities include assisting in all areas of patient care providing factual information based on the diagnosis and the recommendations of the medical personnel.
  11. Patients with a diagnosis of high-risk benign lesions will be evaluated for cancer risk post-surgical intervention. The navigator will assist the PCP in promoting individualized follow-up imaging recommendations.
  12. Offers support in collaboration with other clinicians with clinical treatment guidelines and resources throughout the disease process offering a seamless transition of care.
  13. Collaborates with research team in clinical trials enrollment and data collection.
  14. Identifies patient and/or provider needs and develops quality programs to meet those needs.  Ensures the delivery of quality services for patients.
  15. Revises and monitors educational materials to ensure that they are coordinated, understandable, and meet program standards.
  16. Protects patient confidentiality by promoting professional staff communications.
  17. Collaborate with leadership to provide program evaluation, improvement, tracking metrics, and patient outcomes and report findings to Cancer Committee.
  18. Responsible for NAPBC accreditation as required by each entity.
  19. Meet community educational needs via offsite educational offerings. This could be offered day/evening or weekends.

 

Qualifications:

 

  Minimum Education       Radiology Technologist certified in radiography and

                                      mammography or a bachelor’s degree in a healthcare field

                                     

  Minimum Experience     3 - 5 years radiation experience with at least 1-2 years in

                                      mammography         

                                     

  Req Course(s)/Training American Registry of Radiologic Technologists (ARRT) with current mammography certification in PA                                      

  Physical Demands         Mostly sedentary work

                                      Occasional prolonged standing/walking

                                      Occasionally lifts, positions, pushes and/or transfers patients

                                      Occasional reaching, stooping, bending, kneeling, crouching

 

Working Conditions         Occasional exposure to unpleasant patient or unit elements

                                      Subject to varying and unpredictable situations

                                      Handles emergency or crisis situations

                                      May perform emergency care

                                      Subject to many interruptions

                                      Travel required

                                      Occasional pressure due to multiple calls and inquiries

                                      Occasionally exposed to the risk of bloodborne diseases

                                      May be subject to irregular hours
