General Summary
Provides advanced prehospital and interfacility critical care services during transport of high-acuity patients. Functions as part of a critical care transport team delivering complex clinical interventions in the mobile environment. Ensures safe, protocol-driven, and evidence-based care in collaboration with transport nursing and medical control.
Essential Functions:
- Responsibilities include patient care incorporating assessment, stabilization, and intervention techniques consistent with standards and protocols approved by the Medical Director during critical care transports.
- Executes independent judgment to deliver appropriate care consistent with the clinical protocols when contact with a medical command physician is impossible.
- Provides advanced life support to critically ill or injured patients in adherence to established protocols and standards of care; assesses online medical command or other resources when appropriate.
- Follows PA DOH BLS, ALS, and CCG guidelines while caring for critically ill or injured patients transported by the WellSpan EMS system.
- Manages advanced airway procedures including mechanical ventilation, advanced airway adjuncts, and ventilator troubleshooting.
- Responsible for rendering professional advanced emergency medical and critical care to ill or injured patients in collaboration with an CCRN partner and medical control in the transport environment.
- Collaborates with Critical Care Nurse to provide advanced interventions for complex medical, surgical and trauma patients during transport.
- Performs daily and monthly checks of equipment to assure equipment is in safe operating condition and is mission ready; performs routine restocking of supplies after each transport and maintains established par level inventory; and reports broken/damaged medical equipment to biomedical engineering for repair.
- Maintains clinical competence across all aspects of scope of practice including, but not limited to, cardiac assist devices (IABP, Abio- med, Tandem Heart, Heartmate), ARDS treatment including advanced ventilator management and nitric oxide implementation, ECMO support, obstetric emergencies, trauma patient care across the lifespan, critical care across the lifespan, and any other patient population that is transported.
- Administers and maintains pharmacologic agents and intravenous fluids during transport.
- Performs hemodynamic monitoring during transport.
- Monitors and evaluates invasive and non-invasive procedures and their therapeutic effect, including the monitoring and operation of the fully functional critical care transport ventilator, ventricular assist devices (including, but not limited to, IABP, Abio-med, Tandem Heart, Heartmate), ECMO support, and ventriculostomies. Troubleshoots any problems that arise and makes necessary corrections.

Qualifications
Minimum Education:
- High School Diploma or GED Required
Work Experience:
- 3 years As an EMT Paramedic. Required
- 2 years Experience in Critical Care Ground or Critical Care Flight. Preferred
Licenses:
- Certified Paramedic Upon Hire Required and
- Advanced Cardiac Life Support Upon Hire Required and
- Basic Life Support Upon Hire Required and
- Ped Adv Life Support Upon Hire Required
Driver's License Statement:
Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
Courses and Training:
- Neonatal Resuscitation Program within 90 days Required and
- NIH Stroke Scale Certification within 90 days Required and
- National Incident Management System (NIMS) 100, 200, 700, and 800 within 90 days Required and
- Intl Trauma Life Support Upon Hire Required or
- Adv Trauma Life Support Upon Hire Required
- Nationally Registered Paramedic Certification within 1 year Preferred
Knowledge, Skills, and Abilities:
- Ability for eligibility to receive medical command authorization from the Medical Director.
- Exemplary clinical judgment with critical thinking, analytical, and problem-solving abilities are required for various aspects of patient care.
- Ability to work productively and effectively within a complex environment and handle multiple/changing priorities and specialized equipment.
- Critical thinking skills necessary to exercise and lead others in applying the nursing process.
- Maintains fitness for duty (if qualified as a flight crew member) by adhering to all employee health requirements, including maximum weight, hearing conservation program, and required medical screenings.
