Job Description:

Under the general supervision of the Supervisor-Social Work, processes requests for CBP from patients and families and referral sources.

 

Duties and Responsibilities

  1. Interviews patients and family members to obtain information about home environment, family relationships, behavioral health history, and patient's personality traits.
  2. Evaluates data gathered in terms of the behavioral health plan of treatment, available social service programs (CBP other service programs as needed), and completes referrals to CBP when patient meets criteria, so that each patient will receive services of maximum benefit to patient.
  3. Assists patients and families to understand, accept, and follow medical recommendations and CBP program parameters.
  4. Screens each referral to ensure CBP will provide services in a least restrictive environment for the patient and his/her family.
  5. Interprets nature of emotional or behavioral condition and prescribed course of treatment to patients and families. Communicates family issues to other staff members. Participates in planning conferences.
  6. Assists the discharge team from other programs in developing discharge plans for CBP referrals.
  7. Collaborates with other programs and community agencies with which patients and families are involved.
  8. Prepares and maintains required documentation on each case.
 

Qualifications

  • Minimum Experience:
    • 6 - 12 months
  • Minimum Education:
    • Bachelor's Degree
  • Required Courses:
    • Clearance per Child Protective Services law (11 P.S. 2201) amended by PA Act 33 of 1985
