Job Description


Under supervision of the Senior Vice President-Clinical Improvement & Chief Information Officer or Chief Quality Officer, is responsible for the System-wide patient safety improvement and management program.

Inspires our providers and staff to achieve a level of reliability that results in preventable, harmful events being rare occurrences. Serves as the organizational steward for patient safety by never accepting that harmful outcomes are inevitable and never allowing us to become numb to the consequences of preventable harm for our patients, their families, and our staff members.

Maintains a high level of competency in the science of safety and improvement and leads the System to become a high reliability organization by rigorous adherence to those principles. Serves as the patient safety conscience of the organization and holds us accountable to “first do no harm”.

Provides system oversite to the Patient Safety Officers (PSO) and works in collaboration with physicians and staff in all System facilities to achieve these goals. Supports the PSOs in maintaining compliance with all patient safety related regulatory and legal requirements.


Duties and Responsibilities


Fosters the use of the occurrence reporting system throughout the System Safety Reporting System to minimize blame and retribution and emphasize systems improvement.

Serves as an advisor on any patient safety activities.

Oversees the methodology for investigation and escalation of serious events and, as appropriate, identified incidents. Ensures that any actions as are immediately necessary to ensure patient safety as a result of any investigation are promptly undertaken. Reports actions taken to ensure patient safety as a result of any investigation to the WellSpan Clinical Performance Council/Health Council and the WellSpan Patient Safety Committee. Tracks timeliness and effectiveness of remediation efforts and escalates issues of concerns to senior leadership. Leads cross institutional sharing of RCA learnings to ensure all entities learn from each RCA.

Oversees the System’s responsibilities for timely patient safety reporting to the Pennsylvania Department of Health and the Pennsylvania Patient Safety Authority.

Provides planning and reliability initiatives for the System’s management of product alerts and recalls. In conjunction with Risk Management and Legal departments, employs systematic techniques to assess risk and ensure timely communication of all urgent recalls with potential clinical patient impact, as well as thorough documentation of remediation efforts across departments and service lines, to reduce clinical and regulatory risk.

Leads WellSpan Clinical Equipment Safety Committee, a cross-functional WSH team to ensure safe and efficient review and approval of biomedical and clinical equipment used at any WSH entity. Coordinates efforts of stakeholders from multiple segments of the health system to provide cost and risk-benefit analyses to substantiate decisions regarding clinical equipment. Reports actions taken to ensure clinical equipment safety to the WellSpan Operational Leadership Team.

In a matrixed relationship, oversees and develops the performance of the Patient Safety Officers and practitioners across the System.

Chairs the WellSpan Health Patient Safety Committee and the WellSpan Clinical Equipment Safety Committee.

Oversees, recommends staffing, and provides orientation and scheduling for the System’s team of Just Culture champions and supervises development and deployment of training programs for leadership, physicians and APCs, and frontline staff.

Reviews assessments of compliance with the Joint Commission Sentinel Event Alert recommendations.

In conjunction with staff from the Quality Management and Risk Management departments, suggests areas of focus for improvement efforts, i.e.: systematic techniques such as Failure Mode Effects and Criticality Analysis (FMECA) and root cause analysis for the investigation of failures of the system.

Provides hospital and system trended reports from the System occurrence reporting system and assists the Service Lines and Departments in providing trended occurrence reports to their respective Patient Safety Committees and other committees as needed.

Assesses the System’s safety culture using industry standard assessment tools. Reviews and coordinates assessment and interpretation of results. Works with other members of leadership to develop and implement programs that seek to address issues of concerns from these surveys.

Develops, implements and coordinates patient safety initiatives and goals such as IHI, The Joint Commission, PA Department of Health, and Leapfrog, to support WellSpan’s aspiration to be a healthcare leader in clinical safety.

Reports WellSpan Health Patient Safety information to designated committees and boards as directed by senior leadership.

Ensures that Just Culture algorithms for approaching errors is taught and applied in support of patient safety. Ensures that this and other aspects of WellSpan safety culture are taught at each employee orientation session.

Attends facility/entity Patient Safety Committee meetings and provides consultation for those committees to foster adoption of system-wide patient safety initiatives.

Monitors the external environment for best practices and evaluates these for System implementation.

Provides patient safety related training as needed, including such topics as Just Culture, root cause analysis, event reporting and management, etc.





  • Minimum Experience:
    • 3 - 5 years of clinical or health care experience;
    • Experience in Risk Management/Performance Improvement will also be considered


  • Minimum Education:
    • Bachelor's Degree with a clinical degree such as MD, DO, RN, Pharmacist


  • Required Certification:
    • Current PA license and registration as required (MD, DO, RN, or RPh)


  • Skills:
    • Excellent interpersonal and written and oral communication skills;
    • Excellent organizational skills; knowledge of state and national patient safety regulations.
    • Knowledge and experience with LEAN principles.
