Job Description

 

Under the general supervision of the Director-Clinical Operations, and working as part of a leadership team, is responsible for delivery, efficiency and effectiveness of patient care services delivered by the assigned Branch. Provides supervision and direction for Home Health Clinical Supervisors and participates in activities relevant to the patient services provided. Develops and manages staff and fiscal resources to ensure that high quality, cost effective patient care services are delivered in an efficiently operated system. Assists in evaluation of current service delivery, identifying strategic service needs, and participating in the planning, development and implementation of new programs. May assume the functions of the VNA Director-Clinical Operations in his/her absence, as delegated.

 

Duties and Responsibilities

 

  1. Employs qualified competent staff. Participates in the hiring, orientation, development, coaching, corrective action and terminations of personnel as appropriate with supportive documentation.
  2. Ensures adequate staff education and evaluation by providing direction to educational and teaching efforts, fosters environment for self development and learning of staff, and approving continuing education.
  3. Prepares annual performance evaluations of staff.
  4. Participates in initial orientation program of new employees, as needed.
  5. Ensures adequate, competent and qualified staff to deliver scope of care provided by the agency.
  6. Ensures ongoing process to assess, maintain and improve staff competency.
  7. Organizes and directs the office's ongoing function, assuming responsibility for services and assists in the development and implementation of policies and programs.
  8. Participates in patient care conferences and ensures compliance with standards.
  9. Participates in Performance Improvement activities.
  10. Communicates and problem solves with VNA Leadership on patient care and management issues.
  11. Maintains an awareness of trends and interprets federal and state regulations applicable to all VNA service lines in addition to the state nurse practice act.
  12. Ensures the maintenance of all office records.
  13. Implements policies and procedures.
  14. Supports a Performance Improvement program in place for office operations and complies with Joint Commission standards.
  15. Collaborates with Director-Clinical Operations in identifying potential referral sources.
  16. Establishes and maintains positive working relationships with current and potential referral sources.
  17. Assists in planning, implementing and evaluating field experience of professional students in the home health program, as requested.
  18. Provides assistance in community education, as requested.
  19. Represents the agency in a professional manner with community and professional organizations, as well as System partners.
  20. Participates in committees and in the process of policy and procedure development.
  21. Functions in an efficient cost effective manner with regard to utilization of time, supplies and equipment.
  22. Accountable for seeking consultation, as needed.
  23. Carries out special projects and duties, as assigned.

  4. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
  5. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovations.
 

Qualifications

 

  • Minimum Experience:
    • 3 - 5 years clinical experience
  • Minimum Education:
    • Bachelor’s Degree
  • Preferred Experience:
    • Preferred ExperienceHome Care experience
  • Preferred Education:
    • Bachelor’s Degree or higher
  • Required Certification:
    • Registration and current RN license in PA OR Licensed Physical Therapist OR Occupational Therapist OR Certified Physical Therapist Assistant AND BLS certification
  • Required Courses:
    • Graduate of an accredited program in physical therapy or occupational therapy
  • Skills:
    • Demonstrated knowledge in clinical operations; Ability to exercise leadership as well as to work in a collaborative team relationship with professionals; Ability to accept and utilize administrative direction and consultation; Ability to communicate effectively both oral and written; Possesses good conceptual skills. Ability to use the problem solving approach in assessing, analyzing and planning; Ability and desire to develop professionally and to mentor others; Ability to use lean process improvement to implement and evaluate system changes
    • Must possess a valid driver’s license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
