Job Description

Under the general supervision of the Director-Central Alert and Monitoring Teams, reviews, administers, coordinates and supports the System-wide 24/7 Central Monitoring Team (CMT). Using advanced clinical assessment skills and knowledge with human response to life threatening problems, receives and screens deterioration indicators by real time review of the patient electronic health record for early identification of protocol driven interventions. Utilizing nursing diligence, communicates for action as appropriate, related to potential deterioration and record identified order set and care protocol interventions. Working collaboratively by telemonitoring technology, acts as a real time clinical support for the Rapid Response Team, bedside nursing and/or physician/Advanced Practice Provider on patient conditions that require immediate attention. Is responsible for on-site demonstration as needed, and training and training material development. Supervises on-duty telemetry monitoring technicians and provides assistance and support as needed/requested, under the direction of the Director.

Duties and Responsibilities

1. Performs diligent review of electronic health record systems data and protocol driven alerts for screening of necessary and appropriate interventions, using clinical assessment and knowledge skill and pre-determined/defined order set(s).
2. Provides real time identification and communication of missed opportunity or care gaps related to alert.
3. Provides timely and appropriate communication of identified and related assessment to appropriate clinical staff and/or Rapid Response Team.
4. Prepares professional communication, verbal or written, of identified findings and interventions, including thorough and complete nursing documentation.
5. Maintains a thorough knowledge and understanding of evidence-based/best practice.
6. Utilizes strong decision making, systems thinking and computer literacy skills in a remote practice environment, to assist in support of coordinated and timely patient care intervention.
7. Performs virtual “rounding” regarding patient assignment and alerts, reporting as directed.
8. Follows escalation process/procedure to address real-time care concerns.
9. Revises and monitors educational materials to ensure that they are coordinated, understandable and meet the program standards.
10. Protects patient confidentiality by promoting professional staff communications.
11. In downtime, follows a set procedure for several other job responsibilities such as, review and decision making of incidental findings and the incidental findings registry as a care coordinator, and EWS alerts of various types.
12. Performs Central Monitoring Team quality assurance through interrater reliability scoring and communication

Qualifications

• Minimum Experience:
  • 3 years
  • Min Field of Expertise Nursing experience in Emergency Department, Critical Care, or Remote Monitoring setting/role; Proven experience and competent in rhythm analysis/interpretation
• Minimum Education:
  • BSN or equivalent experience
• Required Certification:
  • Certified Critical Care Registered Nurse (CCRN) or Certified Emergency Nurse (CEN); or other Professional Certification
