Position is based in both Lebanon and Ephrata. Candidates must be comfortable driving to both locations.

Job Description:

Under general supervision, performs a variety of clinical and administrative duties related to clinical staff management, case management, patient care assessment, patient education, quality assessment and direct patient care, in multiple centers throughout a region. Provides holistic assessment, comprehensive wound evaluation and management and coordination of patient care, in all affiliated centers. Plans, implements and evaluates comprehensive wound care involving multiple disciplines and sites of care. Collaborates with the Medical Director to coordinate services and providing quality care to patients with chronic non-healing wounds. Provides direct patient care as needed.

 

Duties and Responsibilities

Essential Accountabilities:

  1. Seeks out and obtains advanced knowledge, credentials, specialty exams, training and education for clinical and administrative role.
  2. Provides continuity of care for patients in collaboration with physicians with accountability for planning, implementation and evaluation of comprehensive wound care involving multiple disciplines.
  3. Functions as a case manager and in other clinic roles and serves as a role model for clinical staff.
  4. Orients new clinical staff to the Wound Care pathway, case management, clinic roles and responsibilities, performance improvement activities and chronic wound management.
  5. Manages and evaluates clinical functions and implements improvement action plans as necessary.
  6. Demonstrates competence in patient and family education including but not limited to etiology, comprehensive wound care, dressings, edema control, specialty foot care and life style changes.
  7. Assumes patient advocate role and protects patient rights.
  8. Functions as back-up coverage for hyperbaric technologist.
  9. Participates with Medical Director in the review of clinic operations and preparation of regular statistical and narrative reports. Ensures the accuracy of all statistical information prior to submission.
  10. Assumes fiscal responsibility of the clinic through collaboration with the Surgical Service Line Business Manager in creation of the budget and independent follow up to maintain operations within the established budget parameters.
  11. Works with the hospital administrative manager to ensure smooth and efficiently run clinics.
  12. Provides supervisory support to the Electronic Health Record (EHR); maintain a working knowledge of the overall system, clinical data input, generating reports, troubleshooting and general policies and procedures.
  13. Responsible for the management and development of clinical staff, functions and activities and appropriate patent care assignments during clinic hours.
  14. Arranges with the Medical Director and hospital nursing management to conduct no less than quarterly medical staff and clinic staff meetings. Maintains documentation for these meetings.
  15. Supports and encourages harmonious working relationships and team building skills.
 

Qualifications

  • Minimum Experience:
    • 3-5 years of management experience
  • Minimum Education:
    • Bachelor's Degree
  • Required Certification:
    • ACLS Certification
