Position Summary:

POSITION WILL TRAVEL BETWEEN YORK, LANCASTER AND LEBANON COUNTIES

 

 

General Summary:

 


Under general supervision of the Manager-Skilled Nursing Facility Relations, functions as part of the Post-Acute Services team to promote clinical coordination and collaboration between acute care entities and various post-acute care sites resulting in safe and efficient care through engaging in and leading a variety of clinical and process improvement projects.

 


Duties and Responsibilities:

 



1 Supports regionally assigned preferred partner and pilot skilled nursing facilities in efforts to drive clinical processes improvement across the care continuum through collaboration with various interdisciplinary teams in acute care hospitals, skilled nursing facilities and the community.
2 Co-leads post-acute services system clinical improvement team.
3 Leads local post-acute clinical improvement teams in assigned WellSpan region and coordinates associated activities and projects necessary for process improvement and implementation of system strategy.
4 Leads clinical education efforts through curriculum development and training staff in each local preferred partner facility and other areas as identified.
5 Drives patient safety efforts through assuming responsibility for the non-WellSpan, post-acute, SRS/CRS RL System Tile for patient safety initiatives with guidance from the Manager-Skilled Nursing Facility Relations.
6 Supports pilot facilities as identified by Post-Acute leadership as critical to the WellSpan clinical and operations strategy. Responsible for working along with SNF leadership to achieve outcome measures, while improving overall quality to a 4-star or higher level.
7 Promotes collaboration between disciplines and departments to improve the care of patients and their families as well as staff competency in caring for common conditions in post-acute care settings.
8 Supports integration of standard workflows related to the electronic health record.
9 Coordinates and implements tactics that support WellSpan's post-acute strategy.
10 Oversees continuous quality improvement through assessing transition of care processes. Uses extensive knowledge, experience, program outcomes and research to identify problem situations or issues in systems of care and analyzes, interprets, advises and consults with staff and other health care professionals to ensure high quality care.
11 Analyzes and interprets the system of patient care delivered by evaluating and reviewing practices and initiating new processes related to professional ethical standards.
12 Makes recommendations for practice improvement and collaborates to ensure high quality of care delivered.
13 Collaborates with other clinical leaders in acute care entities, service lines and post-acute partners to envision projects and services that would enhance patient care across the continuum.
14 Works in close association with WellSpan VNA to promote home health utilization for patients after their acute care stay and post-acute stay respectively.


Required Skills

RN License

BSN preferred

Certified Nursing Director of Long Term Care Certification preferred

3-5 Years of RN experience
