Job Description

 

Under general supervision, performs secondary triage screening to determine appropriate avenue for care of patients. Ensures care of patient by appropriate provider. Monitors access to care. Educates patients and families in home care techniques and the responsibilities of managed care.

 

Duties and Responsibilities

 

  1. Obtains history and gathers triage information to determine appropriate avenue for care.
  2. Verifies patient insurance eligibility and identifies primary care physician.
  3. Relays triage information and facilitates communication between the provider and the patient concerning patient disposition.
  4. Based on assessment, assists physician to determine appropriate disposition.
  5. Assists in the management of patient utilization of health care system and provides patient education relative to health care needs, use of community resources and responsibilities of membership in managed care.
  6. Provides telephone advice according to established guidelines and protocols. Monitors compliance and evaluates effectiveness of home treatment instructions.
  7. Develops telephone triage guidelines as needed.

  3. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
  4. Provides age appropriate assessment and treatment.
  5. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
 

Qualifications

  • Notes:
    • Other combinations of formal education, training and experience may be considered.
  • Minimum Experience:
    • 3 - 5 years (broad clinical practice)
  • Minimum Education:
    • Associate's Degree
  • Required Certification:
    • Registration and current RN license in PA; BCLS certification
  • Advanced triage skills; good communication and assessment skills
