Job Description:

Under the general supervision of the Nurse Manager, provides professional nursing care at a beginner to expert level in accordance with the principles of patient and family-centered care based on the nursing process and according to the clinical standards of the hospital. Coordinates care planning with other disciplines. Precepts or mentors a variety of clinical staff to promote professional development in the department of nursing. Acts as a change agent/liaison/as an extension of nursing leadership to promote nursing department/entity and system level goals and objectives. Safely and independently administer prescribed treatments, medications, and nursing interventions for more complex groups of patients based on assessment and plan of care. May require specialized skills depending on unit assignment.

 

Duties and Responisbilities

  1. Meets all expectations of CNI, CNII and CN III.
  2. Independently completes assessment and collects data on assigned patients according to policy; takes initiative to supplement assessment as more complex issues arise. Develops innovative approaches to solve complex patient care issues. Focuses on important parameters and emerging problems to predict and manage an evolving clinical picture. Establishes contingency plans in anticipation of unexpected events and coordinates emergency action in response to acute risk situations. Demonstrates Leadership at the point of care by actively performing caring behaviors with every patient interaction.
  3. Demonstrates advanced knowledge and skills to safely and independently administer prescribed treatments, medications, and nursing interventions for complex groups of patients based on assessment and plan of care. Provides supervision and acts as a resource person for all expected unit skills and equipment. Independently organizes and prioritizes time based on higher acuity patient assignments and increased workload.
  4. Independently evaluates and monitors the effectiveness of interventions and progress toward outcomes for more complex groups of patients; analyzes data to make recommendations for changes in the plan of care. Initiates and coordinates interdisciplinary collaboration in evaluation of patient progress. Anticipates and coordinates individualized patient needs for transfer/ discharge and includes patient/family in decisions.
  5. Acts as a resource person for other staff with proper documentation and adherence to mandatory initiatives.
  6. Independently reads current literature. Demonstrates inquiry to improve patient outcomes. Participates with a team to evaluate evidence and determine clinical relevance. Directs in collaboration of clinical nurse specialist/clinical educator/staff development specialist as applicable evidence based practice initiatives and projects based upon review of the literature to impact patient and organizational outcomes. Leads, coordinates and facilitates journal clubs at the unit level as applicable.
  7. Seeks learning opportunities and resources beyond own work environment and incorporates into practice the new knowledge and skills acquired. Required education and competencies completed, including 50 or greater CEU’s above system required education. Shares pertinent information and job knowledge, related to readings, educational programs and participation in professional organization activities. Maintains current membership in professional nursing organization.
  8. Collaboratively works with management to monitor the level of professional practice on the unit; assists with the initiation of actions or programs to ensure teamwork and professional practice. Uses advanced leadership skills to promote cooperation, respect and trust in the work environment.
  9. Mentors/coaches others to improve decision-making and accountability. Act as a resource to nurses developing supervising and delegating skills. Precepts or mentors a variety of clinical staff to promote professional development in the department of nursing. Acts as a change agent/liaison/as an extension of nursing leadership to promote nursing department/entity and system level goals and objectives.
  10. Promotes cooperation and communication among all team members. Anticipates the potential for conflict and works to quickly defuse any situations involving conflict with patient/families and/or co-workers. Recognizes multiple factors underlying the conflict situation and chooses and initiates actions that result in the most effective long term solution. In collaboration with management, implements changes to enhance team communication. Coaches team members in developing insights into their own communication patterns.
  11. 11Acts as coach and mentor to CNI, CNII and CN III staff for clinical issues and potential advancement opportunities.
  12. 12Acts as a liaison with other units/hospital entities and system.
  13. 13Maintains cost consciousness and makes recommendations for cost containment through leadership efforts and committees/councils and discussion at the unit/entity/system level.
  14. Coaching and mentoring the use of EBP and NR at the unit level using current professional literature and research formally bringing this to other staff for analysis/discussion/potential practice change to improve care.The following are essential expectations:
  15. Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides . direct patient care to assigned patient age group(s).
  16. Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
  17. Maintains Adheres to established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  18. Enhances professional growth and development through advanced/continuing education to maintain up-to-date knowledge, skills, and competencies.

Tower 3 is a 55 bed telemetry unit which specializes in trauma, neuro and orthopedic care.
