Job Description

Under general supervision of the Heart Failure Program Coordinator and the Heart and Vascular leadership team, coordinates patient care/services from the inpatient setting to an outpatient setting in order to improve patient outcomes.

Duties and Responsibilities

The following are essential job accountabilities:

   1  Introduces patient to the WellSpan Heart Failure Program.

   2  Serves as a liaison between the patients, families and healthcare team during hospitalization and the post- discharge period

   3  Communicates to Cardiologist, HF CRNP, and hospital staff regarding abnormal findings in the evaluation of patients.

   4  Works with the hospitalists, cardiologists, and the HF CRNP in assessing readiness for discharge and risk for readmission.

   5  Responsible for the guidance and coordination of care for patients and their caregivers through the continuum of care for both inpatient and outpatient.

   6  Provides individualized information, support and education.

   7  Works with Case Management and Heart Failure Care Coordinator to coordinate appointments for consultations and physician visits.

   8  Participates in interdisciplinary nursing rounds for inpatients (discharge planning process).

   9  Outpatient responsibilities for patient education and tele management of heart failure patients.

  10  Responsible for PI and other metrics to determine effectiveness with all level of care providers to optimize the patient care experience.      

  11  Maintains a thorough knowledge and understanding of evidence based/best practice.

  12  Revises and monitors educational materials to insure that they are coordinated, understandable and meet the program standards.

13 Protects patient confidentiality by promoting professional staff communications.

Required Education / Experience

  • Current RN license in PA
  • Bachelor's Degree
  • 3-5 years nursing experience
  • Cardiology nursing experience, preferred
