Job Description


Under general supervision of the Cardiac Surgery Program Coordinator, assists in all aspects of treatment for the Cardiac Surgery patient including, support services, patient care/services from the inpatient setting to an outpatient setting in order to improve patient outcomes. Facilitates the appropriate use of medical resources in the inpatient setting using established standards as guidelines. Collaborates with the health care team and the patient /family to accomplish agreed upon outcomes.


Duties and Responsibilities


  1. Introduces patient to the Wellspan Cardiac Surgery Program and the Cardiac Surgery facilities and unit(s).
  2. Serves as a liaison between the patients, families and healthcare team throughout the process of diagnosis and treatment.
  3. Coordinates with the interdisciplinary team to develop a plan of care.
  4. Links patient/family to appropriate organizational or community resources and facilitates access.
  5. Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment.
  6. Provides individualized support to patient/family, during pre-op and inpatient period, as well as after discharge.
  7. Works to coordinate appointments for consultations and provider visits.
  8. Reinforces and assesses pre- and post-operative instructions and provides further education as appropriate.
  9. Communicates and collaborates regularly with physicians and health care personnel involved in the patient’s care across the continuum of care, in order ensure continuity of care and implementation of appropriate care models.
  10. Assists in developing quality programs to meet those needs. Ensure delivery of quality services for patients.
  11. Maintains a thorough knowledge and understanding of evidence based/best practice.
  12. Protects patient confidentiality by promoting professional staff communications.
  13. Works with case management to make arrangements for discharge needs (SNF, Cardiac Rehab, Home Health) in collaboration with patients, families and care team as needed.
  14. Collaborates with physicians, other health care provider, and social work to ensure continuity of care and implementation of appropriate care models

  2. Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment.
  3. Acts as a resource to other healthcare professionals in the practice setting on disease and case management topics and operational approaches
  4. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  5. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

  • Minimum Experience:
    • 3 - 5 years nursing experience
  • Minimum Education:
    • Bachelor's Degree
  • Required Certification:
    • Certification in appropriate nursing specialty; Cardiology nursing experience
    • preferred
  • Physical Demands:
    • Mostly sedentary work
    • Occasional prolonged standing/walking
    • Occasionally lifts, positions, pushes and/or transfers patients
    • Occasional reaching, stooping, bending, kneeling, crouching
  • Skills:
    • Proven outstanding interpersonal skills; demonstrated ability to function as a team player
