General Summary: 

Manages the aftercare planning of an assigned group of clients by collaborating with case counselors, therapists, and psychiatrists to complete details of the discharge plan and make modifications, and to facilitate the discharge of clients from the day hospital program.

 

Duties and Responsibilities:

1 Evaluates data gathered during the admission process to understand the treatment and discharge needs of clients.

2 Communicates the status of discharge planning to the treatment team as updates become available throughout the client's treatment.

3 Promptly reports difficulties or barriers to discharge planning to the treatment team and assigned case counselor or therapist.

4 Consults with treatment team and assigned case counselor or therapist regarding complex discharge planning as needed.

5  Maintains on-going professional relationship with personnell from schools, public agencies, and mental health and drug and alcohol treatment facilities.

6 Facilitates interagency meetings 

7 Contacts personnel in public and private institutions and agencies to make referrals and to arrange and confirm aftercare appointments and other follow-up arrangements.

8 Arranges services that are authorized by insurance or managed care company.

9 Meets with clients and/or familiies to provide information, determine ability of patients to cooprate with the aftercare plan, and to coordiante and finalize the details of aftercare.

10 Contacts clients and families post-discharge to provide support, encourage attendance of aftercare appointments, and encourage participation in aftercare services.

11 Documents efforts being made to assess, develop, and arrange the aftercare plan; document details of contacts with clients, families, and personnel from schools, public agencies, and mental health and drug and alcohol treatment facilities.

 

Required Skills
  • Bachelor's degree in psychology, social work, or human services related field required.  
  • Two years of experience in child and adolescent and or adult psychiatric treatment preferred.
  • Valid driver's license
