General Summary:
Under the general supervision of the Supervisor-Practice Support, works in a collaborative manner with the Quality and Innovation Team, Associate Medical Directors-Quality and Innovation, Quality Improvement Teams, Health Coaches, Process Improvement Coaches, Practice Managers, physicians and clinical support staff to integrate Patient Centered Medical Home/Neighborhood (PCMH/N) into operational work flows. Assesses, plans, implements, coordinates, monitors and evaluates spread of PCMH/N at the practice level to promote high quality, cost effective outcomes. Develops necessary training to ensure adequate support of practice staff. Acts as a change agent and a champion for change.

Duties and Responsibilities:
1 Assists in the implementation, monitoring and ongoing improvement of PCMH/N. Tracks progress on PCMH/N goals, productivity and performance standards. Continuously strives to improve operational efficiencies and workflows. Assists in presenting during Collaborative, LIFT and Clinical Coordinators meetings.
2 Serves as a resource to individual practice leadership Quality Improvement teams and staff in implementing planned care in WMG practices.
3 In collaboration with WMG Education, assists in development of necessary education to ensure adequate support of provider and patient including, but not limited to, developing a training program for the care team and developing education materials for support staff to help support and educate patients on self management.
4 Supports linkages with WSH Clinical Effectiveness Teams to ensure that standing orders and guidelines are up to date.
5 Has primary responsibility for the content and placement of best practice documents on the Planned Care sharepoint portal.
6 Provides administrative support to ensure efficient, effective operation of practices working towards PCMH/N.
7 Educate clinical coordinators on how to perform audits and assessments for clinical staff advancement to POCA II and supports other aspects of quality and innovation monitoring activities.
8 Works in tandem with the Process Improvement Specialists that support Quality and Innovation, to apply lean principles in transforming care in the WMG practices.

  • 1 year experience working in a Patient Centered Medical Home/Patient Centered Medical Neighborhood practice model
  • Must possess a valid driver’s license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
