Job Description:

Under general supervision, Level I Pharmacists provide prescribed pharmaceuticals and drug information as needed for patient care according to professional standards and practices. The Level I Pharmacist role includes routine distributive and clinical services with time in those domains contingent upon the needs of the facility and department. Level 1 Pharmacists may advance to a Level II Pharmacist position based upon job performance and successful completion of the requirements for advancement established in the Pharmacist Career Ladder Program (PCLP) criteria grids.

 

Duties and Responsibilities

  1. Maintains the requirements located in the PCLP Mandatory and Optional Criteria Grids for the Level I Pharmacist.
  2. Provides oversight of safe, accurate, and timely medication order verification including clarification of any medication orders with discrepancies prior to dispensing
  3. Provides pharmacy management and oversight of:
  4. Integrity and security of medications in all care locations
  5. Investigational drugs
  6. Preparation of injectable chemotherapy
  7. Medication wastage, returns, and recalls
  8. Reconciliation of controlled substance wastage
  9. Compounding of formulations
  10. Automated dispensing cabinets
  11. Medication label components and label quality
  12. Provides medication therapy management services in collaboration with other members of the health care team including consultative services pursuant to the facility’s available protocols which may include:
  13. Pharmacokinetic evaluation, monitoring, and dosing (initiation and daily follow-up)
  14. Total parenteral nutrition (TPN) consults
  15. Anticoagulation initiation and follow-up pursuant to a consult
  16. Renal dose adjustments
  17. Intravenous to oral conversions
  18. Provides educational services to patients and healthcare providers regarding medication therapy and/or medication safety concerns
  19. Provides medication event prevention, monitoring, and reporting with an increased emphasis on patient safety
  20. Utilizes technology to enhance patient safety by ensuring the proper distribution, monitoring, and use of medications
  21. Adheres to WellSpan Health’s medication-use policies and standard work.
 

Qualifications

  • Minimum Education:
    • Pharmacy Bachelor's Degree
  • Preferred Experience:
    • Preferred Experience 6 - 12 months hospital experience
  • Required Certification:
    • Valid Pennsylvania Pharmacist License or eligible for reciprocity
  • Required Courses:
    • Completion of 5-year program from accredited school of pharmacy
