General Summary

Level IV Pharmacists have the expertise and advanced knowledge to be viewed as a system-level content expert in either an operational or clinical domain. Their advanced knowledge and practice experience facilitate the delivery of safe, effective, and appropriate medication therapy to their assigned patient populations. Level IV Pharmacists actively participate in interdisciplinary committees, staff education, professional development, performance improvement initiatives, and oversight of pharmacy residents when applicable. Level IV Pharmacists will declare either an operational or clinical focus dependent upon the opportunities available at their facility.Level IV Pharmacists with a clinical focus have a dedicated clinical role with opportunities to manage and monitor medication therapy as part of a direct patient care team, usually in a specific patient population. In addition, they will play a major role in the education of pharmacy students, pharmacy residents, medical residents, providers, nurses, and ancillary staff.Level IV Pharmacists with an operational focus will be highly involved in managing distributive workflow and utilizing pharmacy technologies. They will provide coaching and mentoring to Level I-III Pharmacists, pharmacy technicians, and ancillary staff and play a critical role in the safe and pharmacoeconomic distribution of medications with an increased emphasis on evaluating system cost-savings, formulary management, and regulatory compliance.

Essential Functions:

  • Maintains the requirements located in the PCLP Mandatory and Optional Criteria Grids, the Level IV Pharmacist.
  • Provides oversight of safe, accurate, and timely medication order verification including clarification of any medication orders with discrepancies prior to dispensing
  • Provides pharmacy management and oversight of: - Integrity and security of medications in all care locations - Investigational drugs - Preparation of injectable chemotherapy - Medication wastage, returns, and recalls - Reconciliation of controlled substance wastage - Compounding of formulations - Automated dispensing cabinets - Medication label components and label quality - Procurement and inventory management - Quality control assessment - Coaches and mentors pharmacy technicians and Level I through Level III pharmacists while staffing in the distributive domain - Pharmacy operations as a pharmacist-in-charge (if applicable) - Validates regulatory compliance
  • Provides medication therapy management services in collaboration with other members of the health care team including consultative services pursuant to the department-s applicable services which may include: - Pharmacokinetic evaluation, monitoring, and dosing (initiation and daily follow-up) - Total parenteral nutrition (TPN) consults - Anticoagulation initiation and follow-up pursuant to a consult - Renal dose adjustments - Intravenous to oral conversions - Additional clinical opportunities may be available based upon departmental needs, clinical experience, and performance, and may include unit-based pharmacist, antimicrobial stewardship, patient care rounds, or pain management team - Direct patient care as part of interdisciplinary team, if applicable, through development of individualized treatment and monitoring plans for patients - Assists with medication reconciliation during all changes in level of care, including but not limited to, transfers and postoperative care
  • Provides educational services to patients and healthcare providers regarding medication therapy and/or medication safety concerns.
  • Participates in the training and education of pharmacy technicians, pharmacy students and/or residents, and medical residents. Fulfills and maintains American Society of Health-System Pharmacists (ASHP) preceptor criteria for accredited residency programs, including participation in ongoing departmental preceptor development.
  • Provides medication event prevention, monitoring, and reporting, with an increased emphasis on patient safety.
  • Utilizes technology to enhance patient safety by ensuring the proper distribution, monitoring, and use of medications; participates in the assessment and maintenance of technology.
  • Participates in quality, safety, and performance initiatives related to medication use at the organizational and departmental levels. Takes a lead role in collaboration on departmental process research activities, including mentoring of other staff through research initiatives.
  • Adheres to WellSpan Health-s medication-use policies and standard work. Develops and modifies polices and standard work related to the use of medications within the health system. Participates in the health system-s formulary process, ensuring an appropriate formulary system is in place. Oversees non-formulary medication review.
  • Actively participates on department, facility, or health-system committees, including the Pharmacy and Therapeutics Committee and Drug Shortage Committee and proposes drug shortage management plans.
  • Leads or collaborates in clinical or operational system/process research activities. Contributes to practice literature.

  • Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides patient care to assigned patient age group(s).
  • Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
  • Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
  • Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications

Minimum Education:

  • Bachelors Degree Pharmacy Bachelor's Degree Required

Work Experience:

  • Completion of PGY-1 residency with 3 years of hospital experience. Required or
  • PGY-2 residency with 1 year of hospital experience. Required or
  • 7 years Hospital experience. Required or
  • Internal advancement from Level III Pharmacist position. Required

Licenses:

  • Licensed Registered Pharmacist Upon Hire Required and

Courses and Training:

  • Board of Pharmaceutical Specialties (BPS) certification. Upon Hire Preferred
