Job Description:

Under general supervision, provides prescribed pharmaceuticals and drug information as needed for adequate patient care according to professional standards and practices.

 

Duties and Responsibilities

  1. Reviews all nonemergent orders prior to the first dose administered. Monitors patient medication profiles daily. Reconciles medications during all changes in level of care, including transfers and postoperatively.
  2. Provides initial pharmacokinetic evaluation for narrow therapeutic index medications. Oversees compounding of orders for parenteral nutrition solutions. Provides drug information to the health care team. Provides programs for patients that prevent disease and improve health.
  3. Provides Pharmacy management and/or oversight of: a) medications in all care locations; b) all investigational medications; c) preparation of injectable chemotherapy; d) medication waste, returns and recalls; e) the process of reconciling controlled-substance waste; f) compounding of formulations; and g) automated dispensing cabinets. Standardizes medication doses to improve safety and efficacy. Ensures appropriate medication label components and label quality, and safe medication storage.
  4. Provides medication event prevention, monitoring and reporting. Provides education to allied health professionals on medication safety issues.
  5. Ensures that technology provides patient safety. Uses technology and information systems to ensure the proper distribution and use of medications. Participates in the assessment of technology.
  6. Participates in quality, safety and performance initiatives related to medication use at organization and department levels. Applies infection-control principles to medication dispensing and administration.
 

Qualifications

  • Minimum Experience:
    • 6 - 12 months
  • Minimum Education:
    • Pharmacy Bachelor's Degree
  • Required Certification:
    • License and registration as required
  • Required Courses:
    • Completion of 5 year program from accredited school of pharmacy
  • Physical Demands:
    • Occasional prolonged standing/walking
    • Occasionally lifts supplies/equipment
