PRN (Various Shifts - On Call As Needed) 



General Summary:


Under the direct supervision of the Nurse Manager or designated RN supervisor, attends to patients who are at risk for falls and/or at risk for pulling out IV lines/tubes, in order to avoid the use of restraints.  This position will perform basic patient care activities and related services necessary to observe patient behavior and maintain patient safety while meeting the patient's personal needs and providing for patient comfort.  Position is responsible for following all safety and infection control precautions, instructions and guidelines established for the patient. Position entails contact with patients under wide variety or circumstances.


Duties and Responsibilities:


  • Attends to patients who are at risk for falls and/or at risk for pulling out IV lines/tubes, in order to avoid the use of restraints.
  • Observes and reports changes in patients mental, physical or emotional conditions and calls appropriate staff for patient assistance when appropriate.
  • Provides basic patient assistance with ambulation and mobilization activities safely, including but not limited to simple toileting, re-positioning to provide comfort and assistance with feeding/fluids. 
  • May participate in the provision of diversional activities such as playing cards, board games, reorientation, etc.


Required Skills


  • High School Diploma or GED preferred
  • Excellent interpersonal and oral/written communication skills
  • Must work cooperatively with other team members within the department 
  • Current experience in a hospital or clinical setting providing patient care and/or observation services is preferred.  May consider other relevant experience involving care of the elderly, disabled and/or in incapacitated, children, etc., as well as other community volunteer/service experience 
  • Requires the ability to successfully complete the on-the-job training
