Position Summary:

General Summary:

 


Under the general supervision of the Director-Clinical Support Services, coordinates, communicates and predicts impacts to bed management, transfers and admissions from all possible points of entry and provides general oversight of patient placement and patient flow throughout the Hospital/Health System.

 

 


Duties and Responsibilities:

 

 


1 Provides patient flow coordination, management and optimization 24/7 and coordinates all associated activities with Physicians, Providers and other Clinical Personnel, to ensure timely patient placement based on the patients care needs, diagnosis and acuity.
2 Promotes cooperation and effectively communicates to appropriate professionals throughout the hospital/health system to achieve the objectives of the patient logistics department regarding the improvement of patient flow and urgency for placement and movement of patients during elevated capacity situations.
3 Prioritization and utilization of guidelines to accurately predict and manage patient flow based on the needs, diagnosis and acuity of all patients through all points of entry within the facility and Health System.
4 Coordinate and facilitate direct admissions and transfers, including making appropriate transportation arrangements for the smooth transition and coordination of care.
5 Uses exceptional clinical judgment, based on acquired nursing skills, knowledge base and education to assess patients care delivery needs, acuity, diagnosis and required specialty services in relation to unit placement and available clinical resources.

 



Department Description:


The Department of Patient Logistics provides service to Wellspan York Hospital for patient placement, inter and intra-facility transfers, direct admissions, patient placement, registration, and electronic visit updates. Formed in 2008, the department employs a growing team of Registered Nurses and Administrative Specialists to oversee and collaborate with clinicians and providers to expedite patient flow and patient throughput in the acute care environment. Through the use of multiple software applications, including the electronic medical record and Tele-tracking systems, our nurse coordinators provide clinical review and supportive decision making to determine the most appropriate and safe environment for patient placement. Along with supporting our nurses and organization, our administrative personnel provide excellent telephone customer service support and oversee financial updates in our ADT application for the entire health system.

Required Skills

Associate's Degree in Nursing

Registration and current RN license in PA

Required Experience

2 - 5 years clinical nursing experience in a highly paced acute care environment (may be waived for equivalent experience)

Supervisory and/or charge experience considered essential
