General Summary:

Under the general supervision of the Manager-Continuing Care Services and the direct supervision of the APC (Hospital at Home), is responsible for the daily supervision, coordination and delivery of care to Hospital at Home patients.  Together with the nurses, social workers and care providers, assists in care plan development and implementation, provides advocacy to patients, and helps develop care management strategy for enrolled patients.  Functions within the PA EMS Act scope of practice.

Duties and Responsibilities:

  • Participates in planning for care management.  Coordinates care plans and completes tasks as necessary to achieve care plan goals.
  • Acts as a liaison between the emergency department, specialists, Primary Care Physicians (PCP), community resources and managed care/insurance plans on behalf of enrolled patients to ensure patient-centered coordination of care. 
  • Engages in coaching the patient and caregivers on health literacy necessary to achieve identified goals.
  • Provides professional and thorough patient assessments and treatment services to all individuals in accordance with PA EMS Legislation, Regional protocols, WellSpan Health policies and procedures, and medical direction (when needed).  
  • Effectively communicates patient assessment, treatment and status to other health professionals.
  • Remotely monitors patients within established vital sign parameters and documents findings. Provides appropriate support. Intervenes and escalates care as appropriate. 
  • Accurately and thoroughly completes all necessary documentation in a timely manner and treats all such information as confidential.  Enters and maintains electronic records, compiles reports and completes other program documentation in a timely manner (e.g. progress notes, incident reports, letters, etc.).
  • Participates in interdisciplinary case conferences/team meetings at WellSpan Health locations. 
  • Plays a consistent and active role in identifying project inefficiencies and finding collaborative solutions to the problems.
  • Participates in the education of nursing and medical students that may rotate through the program, as applicable.
  • Immediately reports any unsafe or safety concerns to the Manager-Continuing Care Services.
Required Skills

Minimum Education:  High School or GED

Required Certification:  Certification as EMT-P
