PRN Social Worker (VNA)

Job Description:

Under general supervision, provides social work consultation to staff, patient counseling and direct social work services to assigned patients in accordance with the agency’s standards and policies.


Duties and Responsibilities

  1. Related to Patient Care
  2. Under Physician’s order, provides direct social casework services and home visits to individuals and families.
  3. Participates in interdisciplinary conferences to identify significant social, economic and emotional factors related to the health problem and assist in formulating the plans of care for specific patients.
  4. Records the social service component of care on the patient’s record, including the identified social problems and treatment plans that have been implemented.
  5. Related to Staff
  6. Provides social work consultation to the staff to enable them to assess the significant social, economic and emotional factors related to the health problem.
  7. Advises the staff on community resources available to meet patient needs and assists in making appropriate referrals.
  8. Assists in the orientation of new staff or continuing education of existing staff as requested.
  9. Assists in identifying staff development needs related to family dynamics or social-economic factors.
  10. Reviews and co-signs clinical progress notes written by BSW.
  11. Related to Corporate Management
  12. Submits clinical documentation and records in an accurate, legible and timely manner consistent with agency policy. Meets or exceeds established productivity.
  13. Actively seeks consultation or direction from management personnel as appropriate.
  14. Participates in staff development and in-service programs, as requested.
  15. Related to Community
  16. Maintains current knowledge on community resources and develops a referral network system.
  17. Represents the VNA at community meetings as delegated by clinical management.
  18. Promotes a positive corporate image.




  • Notes:
    • Must be degreed in social work or social welfare per PA Act 68 of 2008


  • Minimum Experience:
    • 12 months experience in health care


  • Minimum Education:
    • Master's Degree from a school of social work accredited by the
    • Council on Social Work Education


  • Preferred Experience:
    • Preferred Experience: Community health experience; computer skills


  • Required Courses:
    • Clearance per Child Protective Services law (11 P.S. 2201) amended by PA Act 33 of 1985


  • Skills:
    • Commitment to and knowledge of patient care in the home and community services and
    • delivery system.
    • Ability to communicate effectively both verbally and in writing.
    • Knowledge of social work methods.
    • Ability to collaborate effectively with community agencies.
    • Ability to analyze community resources and identify deficiencies which need
    • community action.
    • Ability to develop constructive interpersonal relationships with members
    • of the home health team.
