General Summary:
Under the general supervision of the Orthopedic Service Line Leadership team and the Director of Nursing for Surgical Services, serves as the day-to-day lead of the WellSpan Orthopedic Total Joint Program and supervises the development, implementation and organization of the overall program. Coordinates the efforts between surgeons, Hospital administration, nursing leadership and staff while tracking the progress of the program and identifying areas that require attention. Is responsible for all program management, report generation and review, process improvement, patient satisfaction and administrative activities of the program. Supervises and coordinates all aspects of treatment, evaluation, support services and education for patients.

Duties and Responsibilities:
1 Initiates the clinical program project plans as outlined and establishes and maintains project completion schedules.
2 Collaborates in the development and coordination of the long-range strategic plan for the musculoskeletal product line. Communicates long-range strategic plan and goals to all areas involved in the care of musculoskeletal patients.
3 Ensures that the program meets all applicable state and federal accreditation standards.
4 Introduces patient to the Health System.
5 Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment.
6 Provides individualized information, support and education.
7 Coordinates appointments for consultations and physician visits.
8 Notifies primary care and specialty care providers when patient is admitted to the hospital if applicable.
9 Participates in interdisciplinary nursing rounds for inpatients (discharge planning process).
10 Reinforces and assesses pre- and post-operative instructions and provides further education as appropriate.
11 Responsible for the guidance and coordination of care for patients and their caregivers through the continuum of care.
12 Responsible for PI and other metrics to determine effectiveness with all level of care providers to optimize the patient care experience. Compiles and analyses quantitative and qualitative data, including benchmark data, and shares analysis with the leadership team and senior management.
13 Identifies patient and/or provider needs and develops evidence based quality programs to meet those needs. Ensures delivery of quality services for patients.
14 Collaborates with outreach/education coordinators for all service lines in the preparation and delivery of community and professional education programs. Assists in the development and presentation of educational programs geared toward primary care physicians, surgeons, staff and community.
15 Collaborates with research team in clinical trials enrollment and data collection as applicable.
16 Maintains a thorough knowledge and understanding of evidence based/best practice.
17 Revises and monitors educational materials to insure that they are coordinated, understandable and meet the program standards.
18 Protects patient confidentiality by promoting professional staff communications.

Required Skills

  • Microsoft Excel experience
  • Bachelor's degree is required; however, master's degree is preferred
Required Experience
  • 3-5 years nursing experience in an orthopedic practice or 3-5 years in a coordinator position
