Medication History Specialist

 

Full Time - Day/Evening Shift (10 Hour Shifts)

 

Every 3rd Weekend & Every Other Holiday 

 

 

 

General Summary:

 

Under the direct supervision of the Administrative Manager-ED and the WellSpan Hospitalist team, interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients.  Compiled history information is entered into the patient’s electronic health record for the purpose of facilitating medication reconciliation by the admitting provider.  May perform a variety of functions which impact the medication reconciliation process and patient care.

 

Duties and Responsibilities:

 

  • Collects an accurate best possible medication list of the patient’s home medications in anticipation of hospital admission.
  • Gathers information for best possible medication list from various resources (patient interviews, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities) to ensure a complete and accurate entry into the hospital’s electronic health record.
  • Effectively communicates with other health care professionals to prioritize patient care and identify patient specific needs.
  • Accurately enters the home medication list into the Hospital’s information system for utilization by the provider.
  • Updates the medication list in a timely manner so it can be used as the foundation for the patient’s admission medication orders.
  • Directs any unresolved issues to the admitting provider.
  • Greets, directs and assists patients, visitors, physicians and ancillary department staff.
  • Answers telephones using personable etiquette. Answers inquiries by providing accurate information or forwarding callers to the appropriate resource.  Identifies emergency messages, directs information to the appropriate personnel and maintains confidentiality.
  • Communicates delays/difficulties in reconciliation process with appropriate disciplines.
  • Assists with care and maintenance of department equipment, hardware and supplies.
Required Skills

 

  • High School Diploma or G.E.D.
  • Post High School Vocational/Specialized Training (preferred)
  • Completion of Pharmacy Technician Program (preferred)
  • Familiarity of Medical terminology
  • Knowledge of medications (their uses, dosages and techniques of administration)
  • Excellent interpersonal, oral and written communication skills
  • Excellent patient interview/inquiry skills
  • Intermediate computer skills and the ability to use various software applications at once
  • Critical thinking skills
  • Effective time management skills with the ability to prioritize tasks
Required Experience

 

  • Minimum 6 months experience working in a pharmacy setting within a healthcare environment or retail pharmacy (preferred)
