Position Summary:


General Summary:
Under general supervision, works in a collaborative manner with the Quality and Innovation Team, Quality Improvement Teams, Practice Managers, physicians and clinical support staff to guide WMG practices to success in the value-based care environment. Provides training and fosters adoption of optimized workflows, enhanced care management techniques, improved EHR utilization, Patient Centered Medical Home/Specialty Practice (PCMH/PCSP) recognition and other quality improvement initiatives. Develops necessary training to ensure adequate support of practice staff. Acts as a change agent and a champion for change.

Duties and Responsibilities:
The following are essential job accountabilities:
1 Functions as a PCMH and PCSP subject matter expert and liaison for practices. Assists in the implementation, monitoring and ongoing improvement of PCMH/PCSP. Tracks progress on goals, productivity and performance standards. Continuously strives to improve operational efficiencies and workflows.
2 Assists in the implementation, monitoring, auditing, and ongoing improvement of quality initiatives. Tracks the progress of goals, productivity, and performance standards. Stays abreast of emerging trends and state/national guidelines and standards related to clinical quality and health services for WellSpan Medical Group.
3 Encourages physicians and physician practice leadership and staff to explore the benefits, challenges, and opportunities in improving health outcomes and patient experiences while containing healthcare costs. Assists in avoiding duplication of efforts, minimizing inconsistencies, recognizing and reducing burden on the participants, and developing and deploying strategies that are cost effective and improve EHR utilization and standardization.
4 Continuously monitors and manages the practice transformation process, including but not limited to practice observations of current state, review of findings and concerns, consultation of best practice recommendations, and opportunities for improvement related to optimized clinical workflows.
5 Works and coordinates with practice leadership, providers, and clinical staff to operationalize strategies to improve, including implementation and training of efficient and standardized clinical workflows, as well as efficient use of health information technology to support patient-centered care delivery.
6 Identifies gaps in clinical workflows or processes and works with practice team and operational leadership to achieve expected outcomes.
7 Assists in planning, coordinating, facilitating, and presenting for the LIFT Collaborative and Clinical Coordinators meetings. Supports LIFT cohorts within WMG practices and acts as a resource and change agent for rolling out pilot initiatives, best practices, and standardized workflows.
8 Assist with planning and implementation of performance and quality improvement programs within the WMG practices. Serves as a resource to practices for quality measures.
9 Provides training and coaching on operational best practices related to clinical workflows and quality improvements during presentations, meetings, and on site within WMG practices. 
10 Serves as a liaison between practices and EHR team. Functions as a resource for the integration of clinical workflows utilizing the EHR. Identifies gaps within EHR processes and assists in problem solving and troubleshooting, including suggestion of EHR optimization. Provides on-site support as needed.
11 Works in tandem with the Process Improvement Specialists that support Quality and Innovation, to apply lean principles in transforming care in the WMG practices.
12 Manages multiple initiatives to meet goals and objectives on a defined timeline.
13 Serves on multiple WMG committees, workgroups, and councils as required.

  •  One year experience working in a PCMH/N practice model
