Day Shift

 

General Summary

 

Under general supervision, of the Program Director of Bariatric Surgery/ Medical Weight Management, coordinates all aspects of treatment, evaluation, support services and education for patients.  Coordinates clinical documentation from referring and treating physicians for each patient.

 

Duties and Responsibilities

 

  1  Successfully navigates patients through the Bariatric Surgery program.

  2  Serves as a consistent contact for patient/family throughout the process of moving through the Bariatric Surgery Program.

  3  Provides individualized information, support, and education.

  4  Coordinates appointments for consultations and physician visits, tracks the patients through the Bariatric Program and all Insurance requirements.

  5  Notifies primary care and specialty care providers with any changes in treatment protocol.

  6  Reinforces pre- and post-procedure instructions and provides further education as appropriate.

  7  Maintains two-way communication with the Bariatric Surgery Office staff and Surgeons. in order to improve the process of coordinating care for patients.

  8  Reports outcome of nursing/patient care/coordination activities to Service Line leadership.

  9  Serves as a resource to support groups and other activities.

10  Protects patient confidentiality by promoting professional staff communications.

 

 

Qualifications

 

  

  Minimum Education      Post High School Vocational/Specialized Training

                                   

  Minimum Experience     2- 4 years nursing experience

                                   

  Req Cert/Registration    Current LPN license in PA; BLS certification

 

  Working Conditions       Occasional exposure to unpleasant patient or unit elements

                                  Subject to varying and unpredictable situations

                                  Subject to many interruptions

                                  Travel required

                                  Occasional pressure due to multiple calls and inquiries

                                  Occasionally exposed to the risk of bloodborne diseases

                                  May be subject to irregular hours

  Skills: Proven outstanding interpersonal skills; demonstrated ability to function as a team player
