Position Summary:

 

 

General Summary:

 

 

Under the supervision of the Branch Manager, processes all information and referral calls for intake or refers to other services. Provides visits to patients in facilities prior to discharge as needed.

 

 

Duties and Responsibilities:

 

 

1 New referral coordination.

2 Assists with telephone triage to patient calls.

3 Assures agency intake processes meet applicable local, state or federal licensing/regulatory requirements in addition to agency policies and procedures.

4 Coordinates with agency Clinical Supervisors/Staffing Specialists insuring appropriate staffing coverage for new referrals.

5 Ensures effective and timely coordination of client home care services through the timely completion of required documentation and computer data entry for new intakes, as well as timely transfer of pertinent medical data to client’s physician, therapists and agency staff members.

6 Maintains accurate and comprehensive client medical data throughout the intake process.

7 Notifies Branch Manager/Clinical Supervisor of changes that may affect intake practices.

8 Investigates and takes appropriate actions on client/consumer complaints.

9 Attends weekly IDT meetings as appropriate to insure consistent lines of communication regarding new intakes and existing cases needing staffing coverage.

10 Provide patient visits at facilities prior to discharge for preparation to Home Care Services.

11 Functions in a cost-effective manner in regard to utilization of time, mileage, supplies, etc.

12 Meets or exceeds established productivity and quality assurance standards.

13 Seeks consultation, guidance and direction from professional or administrative staff, as needed.

14 Informs manager of perceived problem areas related to patient care, corporate management, or community relations.

15 Keeps clinical skills current with nursing practice norms.

16 Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).



Required Skills

 

 

QUALIFICATIONS:

 

Note: Other combinations of formal education, training and experience may be considered.

 

Minimum Education High School or equivalent

 

Minimum Experience 1 year; Previous home health experience desirable

 

Req Cert/Registration Current LPN license in PA

 

Req Course(s)/Training Completion of accredited LPN program

 

Travel required

 

Makes home visits
