Lead Medication History Specialist

Job Description:

Under the direct supervision of the Administrative Manager-ED and the WellSpan Hospitalist team, interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to York Hospital. Compiled history information is entered into the patient’s electronic health record for the purpose of facilitating medication reconciliation by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care. Serves as the Medication History Team lead and acts as a direct resource for team members. Reports any issues/concerns arising with the team to the Administrative Manager on a daily basis.


Duties and Responsibilities

  1. Oversees the work of other personnel by scheduling and monitoring work. Performs work of a similar nature.
  2. Conducts daily compliance checks and prepares and distributes MBX reports appropriately.
  3. Performs follow up calls to Nursing Homes, physician offices and pharmacies to ensure the team is giving/receiving proper information.
  4. Performs follow up on delays/difficulties with reconciliation process.
  5. Performs audits in conjunction with pharmacy/hospitalist staff.
  6. Prepares SRS reports for MBX team. Provides resolution and follow-up as needed.
  7. Collects an accurate best possible medication list of the patient’s home medications in anticipation of hospital admission.
  8. Gathers information for best possible medication list from various resources (patient interviews, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities) to ensure a complete and accurate entry into the hospital’s electronic health record.
  9. Effectively communicates with other health care professionals to prioritize patient care and identify patient specific needs.
  10. Accurately enters the home medication list into the Hospital’s information system for utilization by the provider responsible for admitting the patient.
  11. Updates the medication list in a timely manner so it can be used as the foundation for the patient’s admission medication orders.
  12. Directs any unresolved issues to the admitting provider.
  13. Greets, directs and assists patients, visitors, physicians and ancillary department staff.
  14. Answers telephones using personable etiquette. Answers inquiries by providing accurate information or forwarding callers to the appropriate resource. Identifies emergency messages, directs information to the appropriate personnel and maintains confidentiality.
  15. Communicates delays/difficulties in reconciliation process with appropriate disciplines.
  16. Assists with care and maintenance of department equipment, hardware and supplies.
  17. Completes educational requirements as required.
  18. Identifies own learning needs and seeks appropriate education. Participates in meetings, on committees and represents the Emergency Department, York Hospital and Wellspan Health in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.
  19. Attends meetings as required.
  20. Performs other related duties as identified.
  21. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  22.   24. Demonstrates behavior consistent with System mission, vision, goals, objectives and patient care philosophy.
  25. Maintains effective communication and good relationships with other discipline within the facility for the patient’s benefit.
  26. Treats all individuals with respect, courtesy and empathy. Willingly assists others with questions. Introduces self to visitors and offers assistance. Verbal and non-verbal responses are courteous, friendly and caring.
  27. Participates in educational programs and in-service meetings.
  28. Responsible for ordering departmental supplies and checking equipment.
  29. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.


  • Minimum Experience:
    • 3 - 6 month minimum Pharmacy Technician experience
  • Minimum Education:
    • High School or G.E.D.
  • Preferred Field Of Expertise:
    • Experience with medication names, their uses, doses and routes of administration
  • Required Certification:
    • Registration: National Pharmacy Technician Certification and Licensure by State Board of Pharmacy pursuant to House Bill 1250
  • Required Courses:
    • 4 weeks hands-on training in the department; Medical Terminology (Optional)
  • Skills:
    • Excellent communication/interpersonal skills; excellent computer skills and the ability to use various software applications at once

    • Effective time management skills with the ability to prioritize tasks
