Position Summary:

General Summary:



Under the general direction of the Diagnostic Support Team (DST) Providers and the Service Line, the Lay Navigator assists patients with suspicious lung nodule findings in navigating the health care system to obtain a diagnosis or ongoing surveillance per the DST Provider’s plan of care.



Duties and Responsibilities:


1. Assists patients in navigating the health care system through the diagnostic and/or surveillance care plan established by the DST Provider.

2. Communicates with designated patients within established time frames about findings, plan of care and results.

3. Communicates with Primary Care Providers and Referring Providers about patient findings, the diagnostic or surveillance plan of care and results.

4. Schedules diagnostic tests, procedures, and specialty consultations as ordered by the DST Provider and completes pre-authorization and patient benefit verification as needed.

5. Provides patient with instructions for diagnostic tests and procedures and utilizes Teach Back to confirm patient understanding.

6. Assists patient in finding a Primary Care provider when indicated.

7. Educates patients and caregivers on the multi-disciplinary nature of lung nodule diagnostic treatment, the roles of team members, and what to expect from the health care system.

8. Identifies appropriate resources (practical, social, physical, emotional, spiritual) for patients and caregivers as needed.

9. Maintains comprehensive, timely and documentation of patient interactions.

10. Maintains all required reports, records, statistics, etc.

Required Experience

Minimum Education:


Associate’s Degree or certificate of completion in a clinical healthcare program



Minimum Experience:


2 - 3 years in healthcare setting



Preferred Cert/Registration:


Certificate of Training for Lay Navigation; WellSpan will provide training as needed after hire.
