General Summary:

LPN, provides case management to patients across the Life Span.  Provides psychosocial support to providers and families of The Well Span Family Medicine N. 4th St Office and ELCO Family Health Center. Works a fair and flexible work schedule which meets the needs of the department.  Completes all other duties as assigned.

 

Duties and Responsibilities:

The following are essential job accountabilities:

   1   Provides age appropriate assessment, intervention and case management to children, adolescents, adults by assessing the psychosocial, environmental, age specific, and financial impact of illness on patients and their family system; documents evaluation, treatment and discharge plans.

   2   Provides ongoing patient care management reflective of plans developed with patient/family/health care team.

   3   Effectively acts as a liaison between patient, family and health care team and ensures that patient issues regarding care are adequately addressed.

   4   Provides education to residents to assist them in promoting wellness with a diverse patient population.

   5   Conducts home visits as part of the healthcare team.

   6   Participates with the Patient Centered Medical Home initiatives.

   7   Collaborates with community agencies to coordinate services for our patient population

   8   Participates in Community Initiatives

   9   Engages in Patient outreach

10   Provides Patient education regarding chronic conditions

The following are common expectations:

11   Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population.  Assesses and interprets patient age specific data and provides appropriate, age specific treatment.  Provides direct patient care to assigned patient age group(s).

12   Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.

13   Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.

14   Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Required Skills

  Minimum Education             Post High School Vocational/Specialized Training

                                              

  Minimum Experience            1 - 2 years

                                              

  Req Cert/Registration           Current LPN license in PA; BCLS/ACLS/PALS certification, as required; Basic Life Support (BLS) certification required

 

  Req Course(s)/Training        Completion of accredited LPN program; rhythm recognition course per unit of assignment
