Job Description:

Under the general supervision of the Manager-Continuing Care Services and direct supervision of the APC (Hospital at Home), provides intake and scheduling duties for Hospital at Home patients. Coordinates care delivery with appropriate Hospital at Home staff, Connected Care at Home staff and other ancillary services, as needed.

 

Duties and Responsibilities

Essential Accountabilities:

  1. Coordinates new referrals.
  2. Ensures agency intake processes meet applicable local, state or federal licensing/regulatory requirements in addition to agency policies and procedures.
  3. Schedules visits for all Hospital at Home patients.
  4. Ensures effective and timely coordination of Hospital at Home services through the timely completion of required documentation and computer data entry for new intakes, as well as timely transfer of pertinent medical data to client’s physician, therapists and other staff members.
  5. Maintains accurate and comprehensive client medical data throughout the intake process.
  6. Notifies supervisor of changes that may affect intake practices.
  7. Investigates and takes appropriate actions on client/consumer complaints.
  8. Attends meetings as appropriate to ensure consistent lines of communication regarding new intakes and existing cases needing staffing coverage.

  5. Keeps clinical skills current with nursing practice norms.
  6. Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
 

Qualifications

  • Notes:
    • Other combinations of formal education, training and experience may be considered.
  • Minimum Experience:
    • 1 year; Previous intake/scheduling experience desirable.
  • Minimum Education:
    • High School or equivalent
  • Required Certification:
    • Current LPN license in PA; BLS
  • Required Courses:
    • Completion of accredited LPN program
  • Physical Demands:
    • Moderate physical effort (lift/carry 11 to 25 lbs)
    • Mostly sedentary work
    • Occasionally lifts, positions, pushes and/or transfers patients
    • Lifts supplies/equipment
    • Considerable reaching, stooping, bending, kneeling, crouching
  • Skills:
    • Demonstrated clinical, interpersonal and communication skills; Strong assessment and organizational skills; Ability to multi-task and work under time constraints; Proactive approach to intake management demonstrating leadership skills at all times. Willingness to request and accept consultation and direction; Ability to maintain confidentiality of patient and organizational information
