Job Description:

Provides supportive services for patient requiring anti-coagulation or high risk medication therapy in accordance with the principles of patient and family-centered care according to the clinical standards of the service. Acts as a primary resource for patients under going anti-coagulation care and/or on high risk medications. Coordinates care planning with other disciplines. May require specialized skills depending on individual patient needs.

Duties and Responsibilities

  1. Provides and coordinates direct care in accordance with the principles of medication safety according to the clinical standards of the service both in face to face and telemanagement approaches.
  2. Safely and independently administers prescribed treatments, medications, and nursing interventions to enhance the quality of care in medication use and avoid harmful side effects. Follows protocol driven algorithm to ensure proper dosage adjustment.
  3. Competently uses point of care (POC) technology to obtain INR results (fingerstick blood results) that determine patient’s current therapeutic range.
  4. Provides in depth patient education at initiation of high risk medications and ongoing support to patients and caregivers. Appropriately organizes and prioritizes time based on patient condition and expected workload.
  5. Is able to identify at risk patients and elevate issues using critical thinking.
  6. Identifies and communicates individualized patient needs for safe handoffs during transitions of care. Collaborates with patient/family in care planning with all disciplines.
  7. Appropriately and correctly documents the patient response to interventions and progress toward outcomes. Identifies and documents variances. Appropriately utilizes flow sheets, progress notes, and/or electronic medical record for documentation.
  8. Identifies own areas of professional growth and development in performance improvement efforts. Recognizes the value of evidence based research as it applies to best practice.
  9. Delegates aspects of care to other nursing and support personnel in order to meet patient needs. Helps assistive personnel organize and set priorities for work. Monitors completion of tasks that are assigned or delegated to the patient care team.
  10. Promotes collaboration and cooperation among physicians, nursing staff, and other members of the interdisciplinary team. Contributes to effective communications among all team members. Begins to display conflict resolution and negotiation skills to maximize team performance.

Required Education / Experience

  • 2-3 years cardiology or anticoagulation medication safety experience
  • LPN license in PA
  • BLS certification
