WellSpan Endocrinology

At WellSpan Endocrinology in York, we work as your partner in diagnosing and treating diabetes, hormonal disorders, growth and puberty-related disorders, osteoporosis, menstrual disorders, hypogonadism (low testosterone levels), polycystic ovarian disorder (PCOS), and disorders of the thyroid, adrenal gland, parathyroid, pituitary gland and lipid system.

Job Description

Under the general supervision of the Service Line Administrative team or Practice Manager, provides group and individual Diabetes Education. Provides nutritional counseling to clients referred by a provider. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan with each program participant.

Duties and Responsibilities

  1. Provides group education and individual education in Diabetes Education.
  2. Provides nutritional counseling to clients referred by a provider.
  3. Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSMT program participants.
  4. Manages blood glucose for patients in Endocrinology.
  5. Manages insulin pumps and continuous glucose monitors for patients in Endocrinology.
  6. Collects DSMT program participant assessment data, using the ADDE7 framework, in a collaborative and ongoing manner.
  7. Collaboratively develops an individualized follow-up plan with each program participant.
  8. Conducts a follow-up assessment upon completion of DSMT program services, using outcome measures from the AADE7 Continuum of Outcomes Framework.
  9. Documents assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records.
  10. Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
  11. Participate in peer review process to evaluate performance of other professional staff.
  12. Able to lead and manage groups.

Required Education & Experience

  • Minimum Experience:
    • 2 – 3 years
  • Minimum Education:
    • Bachelor's Degree
  • Required Certification:
    • Registration and current Licensed Dietician Nutritionist (LDN) license in PA; CPR certification
  • Preferred Certification:
    • Certified Diabetes Educator
